Question: To Rakhl Tayal, M>D> Hope you are well From med record 37 wk female induced for decreased fetal movement for following reason: Fetal Assessment demonstrates: 2 accelerations in 30 mins OR a BPP of 8/10. No late deceleration in the past 30 minutes. No variable deceleration's exceeding 30 seconds and decreasing greater than 60 bpm from baseline in past 30 mins. ( Baby had no problems during pregnancy; normat fetal movements and 10/10 up until two days prior to inducement): Does this finding justify early inducement no other tests done. US NST etc Thank you

The findings of non stress test are normal. But a BPP of 8/10 is suggestive of fetal compromise. This can be due to decreased fetal movements or a decrease in liquor. Waiting for 2-3 more days could have caused a further decrease in BPP. This justifies the early inducement.

Thank you Could there have been any test that would. confirm this condition prior to inducement. Patient came to hospital at 6:30 PM for routine sonogram (never performed) this condition was found at 11:30 PM inducement began 12:15 born 12 hrs. later vaginal birth

BPP itself is a test to confirm that there are decreased fetal movements or the fetal body tone is reduced that suggests a compromised blood supply to the fetus. After it was found at 11.30 PM, it was right to induce immediately after.

Thank you for quick reply very helpful. In this matter Biophysical test was not performed. What is BPP test referenced and should there be a printed record of results of it. Is it a subjective (not objective finding) finding by physician?. Throughput pregnancy and up to two days before inducement baby fine, fetal movements normal and 10/10 bio Thanks again

BPP is same as biophysical test.

Detailed Answer:
BPP refers to Biophysical profile. It is done during an ultrasound scan and the result is recorded in the report. The fetal tone and body movements are subjective findings while the amniotic fluid index is an objective finding.
The biophysical profile can reduce due to decreased fetal breathing movements or decreased liquor. It can decrease in 2-3 days also.

You are the best. But as mentioned in this matter there was no ultra sound'; no sonogram or bio physical testing of any kind noted in medical record.. Is there any other way that physician can report the 8/10 BBP and the decelerations noted in original question.? Have great day

Without an ultrasound scan and NST it is not possible to report a BPP of 8/10. Your NST was normal. An ultrasound scan is needed to report BPP.

No. Fetal heart monitoring strip only documents the fetal heart rate accelerations with movement and deceleration with fetal distress. Fetal body tone or amniotic fluid or the frequency of movements cannot be assessed with this.

Since there appears to be no test whatsoever in medical record could you render an opinion on where the doctor would arrive at his assessment regarding 80/10 BP, accelerations etc since he seems to rely on that for his decision to induce early? A BPP of 8/10 cannot be assessed without an ultrasound scan. Only the doctor who put these notes can tell how he came to this finding. The decision to induce early was correct if actually the BPP was 8/10. But this cannot be detected without an ultrasound scan.
