Question : I just had pelvic and transvaginal ultrasounds because I've been having pelvic and back pain and, since I won't get the results from my doctor until the middle of next week, picked up a CD of the images. I am 44 and two years post-menopausal. One pregnancy 7 years ago, have not been on oral contraceptives for 15 years. My right ovary measured more than twice as large as the left (left 6 cc and right 14 cc) and my right ovary is showing a two-sectioned cyst with red and blue color on the dividing line of the cyst. The two sections of the cyst together measure about 2.3 cm x 2.4 cm. Can you tell me what this means?

An ovarian cyst of size 2.3 cm X 2.4 cm with an internal septation is likely to be a benign cyst.
Ovarian cysts are found on transvaginal ultrasound in nearly all premenopausal women and in up to 14.8% of postmenopausal women. The majority of these cysts are functional in nature and benign.

If I were to be your treating doctor, I would suggest the following tests:
1) Serum CA-125 Levels
2) MRI of pelvis to see contents of cyst and density.

Please wait for your follow up to see gynecologist with these reports who can guide you regarding various surgical & non surgical options of treatment.
Usually only wait and watch is needed for the benign cysts.

Thanks, Dr. Tayal - I just uploaded three of the images from my ultrasound, this is the first time I've used this site and it took me a few tries to figure out how to export them. I don't know if those give you any more information or not. Do you have any idea why my right ovary would be so much larger than the left? Is this common? Most of my pain is on the right side and it got worse yesterday when my son (he's 7) jumped up to give me a hug and hung on me and I lifted much of his nearly 60 pounds and felt a sharp pain that hasn't really gone away. Thanks again.

Thanks for writing again.
I have seen the images attached. There is a simple ovarian cyst with a thin septa with no solid tissue in the cyst. It is likely to be a simple cyst with intact vascularity.
The larger size of the ovary is due to the presence of the cyst. It can cause mild stretching of the round ligament causing a dull ache in the area. This is not worrisome. Just go for your follow up and get your tumor markers evaluated.
