Question : I just had pelvic and transvaginal ultrasounds because I've been having pelvic and back pain and, since I won't get the results from my doctor until the middle of next week, picked up a CD of the images. I am 44 and two years post-menopausal. One pregnancy 7 years ago, have not been on oral contraceptives for 15 years. My right ovary measured more than twice as large as the left (left 6 cc and right 14 cc) and my right ovary is showing a two-sectioned cyst with red and blue color along the dividing line of the cyst. The two sections of the cyst together measure about 2.3 cm x 2.4 cm. Can you tell me what this means?

Brief Answer:
unlikely to be ovarian carcinoma

Detailed Answer:
hello,
thanks for contacting health care magic.

looking at your concern, it appears that you are postmenopausal and have been detected with enlarged right ovary measuring 16cc in volume with two cysts separated by a septa.
one's main concern on seeing enlarged ovary in post menopausal age group is to rule out OVARIAN CARCINOMA. there are certain ultrasound features which indicate towards it:- solid components in ovarian cyst, multiple THICK SEPTA (separations), irregular enlargement.
also if ultrasound points towards malignancy , then certain tumour markers need to be done to establish diagnosis like CA 125, CEA, AFP, Beta HCG.
To me , if the dividing line is thin (septa) on Ultrasound and there is no irregularity or solid components, then its a simple ovarian cyst or an endo metrioma which can be laparoscopically removed easily.
i suggest it would be helpful if you can attach the Ultrasound report with the query for further evaluation.

thanks,
regards.

Thanks, Dr. XXXXXXX - I just uploaded three of the images from my ultrasound, this is the first time I've used this site and it took me a few tries to figure out how to export them. I don't know if those give you any more information or not. My HCG test was negative, I have not had any of the others yet. Most of my pain is on the right side and it got worse yesterday when my son (he's 7) jumped up to give me a hug and hung onto me and I lifted much of his nearly 60 pounds and felt a sharp pain that hasn't really gone away. Thanks again.

Brief Answer:
likely benign but rule out malignancy

Detailed Answer:
Hello ,
thanks for the follow up.
i have seen the pictures and to me it appears to be simplae cyst with thin septa and no solid components within, unlikely to be malignant.
the pain is probably due to ligament stretch as in doppler ultrasound there is no evidence of torsion ovary, also vascularity is maintained.
however, in the post menopausal age group it is always advisable to think of malignancy first in terms of ovary unless ruled out by tests.
so advise would be to get CA 125, CEA, AFP and Beta HCG( not for pregnancy but beta hcg tumours) tests done together as tumour markers to rule out confirmatively.A MRI would be more conclusive as compared to Ultrasound.
you can follow up with the test reports.
