What Does Pain While Urinating Indicate As Per The Attached Results?

Question: Hi,
My two and half year daughter is having pain while passing urine and crying during that time.
We have given the urine for testing and below are the results.
Albumin - Present(+)
Sugar - Nil
Red blood cells - 0-2/hpf
Pus cells - 40-44/hpf
Epithilial cells - 2-4/hpf
Crystals - nil
Casts - nil
Other findings - nil

Can you please advise on the results...

Hi,

Thanks for asking on this platform,

The urinary report (presence of pus cells, RBC and albumin) suggests urinary tract infection.
Here I would like to send a urinary culture report to see the causative bacteria and the sensitivity pattern to the antibiotics. Soon after collection of culture sample, the antibiotic treatment should be started.

Oral Cefixime is a good choice for UTI in this age. The dose is 10 mg/kg/day in two divided dosages.Most of the bacteria causing UTI usually so sensitive to Cefixime. Once the culture report comes based on that (if the bacteria is resistant to cefixime) the antibiotic may need to be changed.

Most of the children respond well to this treatment within 3-4 days of starting the antibiotic.
The total duration of treatment should be 7-10 days. Give plenty of water and maintain proper hygiene of the genital area for early recovery.

I would like to get an ultrasound done to rule out any structural abnormality of the urinary tract. DMSA test is also needed in this age to see the structural changes and to find any infection-related damage to the kidney.

Ultrasound should be done as early as possible and DMSA after 3 months.
Get a repeat urinary test done after 7 days to see the response of treatment.

I would suggest you consult a pediatrician for proper work up and follow up to avoid any UTI related complication.
