Something painless on the top of head with tight skin around is most likely a sebaceous cyst. Totally innocuous and nothing to worry about. A simple excision of the structure would help and suffice. Get to your doctor nearby and let them examine the exact nature of this structure with detailed history and physical examination. I am sure they will come up with a plan. Stop worrying, it is probably nothing but better to be assessed and evaluated.
