Question: Found blood mixed with semen yesterday for the first time (visually, anyway)...no pain on ejaculation...asymptomatic, really...seeing Doc this afternoon; what could this be?

Hello and welcome,

Blood in the semen (hematospermia) can be from a number of different regions, most are benign, but at your age it is important to be seen by your doctor for evaluation.

The most common cause of this is infection or inflammation anywhere in the genitourinary system. Usually either in the bladder or prostate. So when you go in to see your doctor, he or she will do a prostate exam, and check a urine specimen for signs of infection or inflammation. If it looks, on a "dip urinalysis" (a quick test done with a color changing dip stick) like an infection, your dr. will start you on antibiotic, but should also send the specimen to the lab for a culture and sensitivity test to make sure this is the right antibiotic.

Uncontrolled blood pressure, chronic liver disease and other systemic health problems can cause hematospermia too, but that doesn't sound like it is likely to be going on with you.

Occasionally very vigorous exercise, vigorous sex, or masturbation can cause it too. The latter two causes because of trauma to the urethra.

An enlarge prostate can cause hematospermia but doesn't usually come on abruptly.

Tumors/growths can be a cause but is rare compared with the other causes I've mentioned. So your doctor should order a PSA, and in addition, if there is no obvious cause like infection found, refer you to a urologist for further evaluation.
