Question: After sex and a swim yesterday I found blood in my underwear. At 11PM it was about the size of a quarter, and it frightened me. I see no sign of blood in the urine. What could this be? It has not continued.

Thanks for your query, albeit short and specific.
To recapitulate: Male/74 - blood in underwear after sex and swim - size of a quarter at 11 pm - no sign of blood in urine - not continued - wants to know further.
The commonest cause of such a blood in underwear, particularly after sex is usually due to congestion of prostate and release of prostatic fluid after ejaculation.
This may be found and not uncommon but surely raises a concern.
Hence I would advise you the following:
Per-rectal digital examination, preferably by an Urologist or a General Surgeon.
Ultrasonography of abdomen.
Blood PSA
Urine
These will help to rule out any serious matter.
If anything is found positive, may need a biopsy.

This is the routine that is usually followed on blood per urethrum.

My prostate was removed exactly 3 years ago--I'm sorry i forgot to mention that. There was no cancer; it was simply swollen. Timing: 3PM hot tub, then sex, then swim for a half hour. There was a little blood on the towel after I showered, but I just thought I had scratched an itch too hard (it was so little). It was 11 PM before I saw the blood in the underwear. The bleeding had certainly stopped and has not recurred. I suspect the bleeding stopped very soon after the swim & shower, but I never felt anything.
