What Does Presence Of Cysts In Liver And Kidney Indicate?

Question: last year i had microscopic hematuria and they ordered a CT of abd /pelv which showed nothing but very small cysts in liver/kidney, which they assured me were common for people my age. i had a hysterectomy 7 yrs ago and loaded with cysts inside..wanted to be sure they were all stable because it scared me, so a year later had a repeat CT. Everything they said was stable w/cysts and was told all was normal and didn't need to come back or f/u. When I got written report, i noticed it said "spleen borderline enlarged" . Dr. never mentioned this, so i went to fam dr. who reveiwed it said it was normal and i didn't have to worry about anything, but i still am. The first one didn't even mention anything at all about my spleen, now a year later this one does. I'm reading a lot of stuff on here that has me worried, and i hate going back and being a nuisance if i really don't have to.

Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
See presence of cysts in liver and kidney is considered important if they are very large in size or associated with any malignant or hemmorhagic changes.
And I assume you had fibroadenomas or uterine benign polyps for which you underwent hysterectomy.
But yes spleenomegaly should be evaluated.
Provide me following details:
1. How many cysts of what size you have in your kidney? Have you been labelled by your doctor ever as having Polycystic Kidney Disease?
2. Give me details that what exactly you had for which hysterectomy was done.
3. Any history of Hypertension?
4. Any history of anemia or reduced platelet count?
5. Any history of hepatitis B or C?
6. Any alcohol consumption? If yes provide me duration and amount?
7. Attach your all reports

Answering your questions:
My CT abd reads: at least 4-5 small low density lesions in the liver, too small to characterized, but likely representing cysts, the largest measruing 6 mm. These are stable from last exam. Spleen borderline enlarged. Small low density cortical lesions in each kidney are also stable, measuring 6 mm on the R and 3 s 10 MM on left. These are also too small to characterize, but of low suspicion. No solid renal masses are identified. IMP: Small low density renal & liver lesions r stable, & likely represent tiny cysts. No solid masses are identififed.

Have no PKD
Hyesterectomy was done because I had a large fibroid cysts, actually two was found, and had numerous intramural leiomyota, ovaries revealed scattered cysts, some hemorrhagic as well as what appears to be scattered fibromata, including a subcasular fibroma.
No Hypertension
7yrs ago had anemia and low platelets found on blood work because I was stressed and not eating. Took iron supplements to bring it up. no problems since. No hepatitis B or C. Have been tested for C
Alcohol consumption is very light. No hard stuff, maybe one beer three times a week. Hardly any wine, maybe 1/2 glass once per month. Have been doing that for about six months.
Was told by both my doctors that nothign was wrong w/having spleen borderline enlarged, and one dr just said that if "here" s the line of normal, mine might be just a tad under that line, but not over it??? I read things like sometmes it enlarges and goes back to normal when it is doing its job. That's all info i have. Thanks you. In other words, I don't think they read it as me having "splenomegaly"

Dear Friend,
Your fibroid polyps of uterus are unrelated to the cysts in liver and kidney.
In fact, the cysts mentioned seems to be benign.
So, normally we don't advise any course of action.
As far as your spleenomegaly is considered, there are many reasons for that. In fact as you have read, it increases for a short time and then gets back to normal state. For example if you had any infection, it might have increased and it would regress back to normal state after some time.
Chronic anemia can also lead to spleenomegaly.
If you want to be very sure, i advise you to get following investigations:
1. Fibroscan of liver - To see for any evidence of fibrosis causing chronic liver disease and enlargement of spleen.
2. Hepatitis B Surface Antigen.
3. Liver Function Test
4. Repeat Complete Blood Count.
5. Alpha Feto Protein - To see for nature of the cyst.
Try to further limit alcohol consumption.
If these investigations also come out to be normal, you don't need to worry at all.
