Brief Answer:
several features

Detailed Answer:
First, it is highly unlikely that a malignancy can be present for one year and not be easily detectable nor have no other complications. Second, pancreatic problems cause pain by interaction with the pancreas. if the pancreas is irritated/hurt/damaged/blocked, then pancreatic enzymes are hugely dumped into the blood and their level gets to be enormous. Third, irritation of any significant amount in the abdomen has many other effects... changes in the tone of the gut, difficulty in digestion/elimination/etc. Cramping of the gut without actual bowel blockage is common and a common cause of abdominal pain and would not show up on tests. Part of the bowel not getting enough blood can also cause problems. Generally, it is detectable and generally it causes serious problems in well under 1 year.
so... minor changes in bowel motion can cause pain. It usually also has some relation to moving one's bowels. There is not great treatment for it.
