Question : I’m a 20 year old heterosexual male, live in the US. I was in XXXXXXX during the summer for a vacation where I had unprotected insertive fellatio with a sex worker and protected vaginal for 2-3 secs. I didn’t know that STDs could be transmitted through Oral sex. I didn’t have any symptoms until 7weeks after the incident. I started with a boil-like bump in groin which I discovered while taking a shower. It was painful and had some whitish pus in it (about 1cm). I drained it quickly the whitish pus came out with some blood. 2days later, while shaving I discovered another in my left armpit, I tried draining it but it was very painful. The next day 2 more appeared under the same arm. I drained them after 5-6 days. Ok, I thought it was over but after a day 4 more pop out, this time with one bigger than the other 3. Then started to get worried and BOOM! STDs came to my mind. I went straight to the internet to google some my symptoms, that was the worst mistake of my life. I read a lot about HIV and became very worried. I couldn’t sleep later that night, the following day I woke up with headache, nausea, dry mouth, loss of appetite, indigestion with excessive gas, muscle and joint pain but there was no fever and night sweats. Then I convinced myself that I had contracted HIV. I became very paranoid, anxious and had suicidal thoughts. Had that for 2days and decided to see Doctor. I was diagnosed with “Axilla Abscess” and was given antibiotics for bacterial infection, however, I didn’t tell him about my exposure thinking that he’d order HIV test which I was scared of. After a week, all the abscesses were gone except the largest one. It flattened (the skin over it peeled off became very painful when in contact with cloth or underarm and it didn’t drain pus though it seemed to contain it) a few days after my medication was over. Then I regained my strength and recover my mental health after talking to an expert online.

9-10 days later, I came down with a dry and itchy throat although it was beginning of dry winter here and I sleep with my windows opened. It became more serious, feeling of something being stuck in there, painful lower jaw, dry mouth, tooth and ear ache, stiff neck and morning headaches. I decided to use the in Home-HIV1/2 test kit after 85days of exposure, it turned out Neg-. To my surprised all my symptoms disappeared after having that result and my severe anxiety was gone. The test is 99.9% accurate if taken outside the window period (12weeks). Ok everything was good for a while not until I read the test could have a false positive and negative. I became paranoid, back to square one. I didn’t sleep that night, next morning I woke up with headache, diarrhea more like indigestion my stomach was filled with gas, loss of appetite and dry mouth. The next day still had the feeling of something being stuck in my throat, this time with a symptom and it is tingling/tickling of muscles including feet, arms, shoulder, buttocks. I took another Oraquick in Home HIV test at 104days. Again Negative. I called AID helpline explained my case and they said I’m conclusively Neg, my risk was low and I took two test outside the window period I was relieved for a week then I read about people testing positive after 6months after testing negative several times. This had me concerned up till now. So I built up the courage and went to a planned parenthood. They tested me using Oraquick fingerstick blood test, the result was Neg. Even today, I’m still concerned about HIV, I dream about it everything when ever I sleep, I wake in the middle of the night and can’t go back to sleep again, I have this burning sensation on my tongue and still have the muscle tingling that seems to come and go. I still have the uncomfortable feeling in my throat.

Now what STD do you think it could be based on the information I provided? Everybody is telling HIV is not my problem but I’m having a hard time accepting that.
And can having indigestion result in acid reflux causing sore throat?
I was told that I should test for Group B Step, Upper respiratory infection etc.

Please try and look at my case in the best possible way.
I’m having a severe anxiety please help me. I need help I don’t wanna continue living in fear.
Or I am having another STRAIN of HIV since my exposure was in XXXXXXX and the tests in the US are not picking it up? Is that possible? But I was told that the test detects antibodies for HIV1/2 though HIV2 is prevalent in XXXXXXX where I had my encounter. I know it obviously seem I’m only concern about HIV, yes I know, I rather it being anything thann HIV honestly. You are my last hope.

Brief Answer:
Negative for HIV.Get Blood tested for HIV.

Detailed Answer:
Hi
Thank you for query. I am Dr Bharatesh D Basti answering your query.
You are getting too anxious about HIV.The symptoms appears to appear when you are anxious.
The oral test has come negative repeatedly is a good sign and sensitivity of test increases with repetition.Repeated test points toward negativity. But it is always better to get your blood tested for HIV.At this stage the results will be conclusive. You can also get tested for other STDs like Hepatitis B and C, and herpes also. These will definitely allay all your anxiety.HIV2 is rare and these are detected even in oral tests. So you need not worry about HIV2.
The symptoms are all due to anxiety or may be due to gastritis or GERD which can cause similar symptoms. You can go for chewable antacid or syrups. Also take H2 blockers like ranitidine.
Just relax and go for tests.
You can get back to me for any clarifications.
Thank you

Hi, Doc. I got another HIV test I just got the result now. Elisa antibody test hiv1/2 Neg. So I’m conclusively neg-. But that’s not all, something is wrong with. Remember the burning mouth I told you about, I just noticed that the taste buds on my tongue have inflamed but less burning throughout today maybe because the tasting buds are inflamed. I have this heated skin and my throat is still itchy. Symptoms points to GERD, but my stomach is better which why I’m doubting it is GERD. I dont know maybe it is Diabetes 1 I read that it causes boil-like skin abscess which I had. My maternal granda has diabetes. But I can;t tell what it is which why I’m here for you a MD to help me figure it out. Thank you.

I have never felt better. Since HIV is out of the picture, I will noyt have problems with anxiety anymore. I’m going to sleep a lot this weekend to recover my strength both physical and mental.
