Question : My husband has been on Androgel for years, due to a low Testosterone. His leval barely reaches a normal limit with this. His PSAs have fluctuated and he has had 3 biopsys His last PSA was 7.4 and his biopsy showed out of 7 biopsys, two cores from each site except one, one showed Adenocarcinoma < 5% in one of the specimens. His XXXXXXX score was 3+3=6(Grade Group 1). The Urologist recommended no treatment, a repeat PSA in 6 months and a biopsy and or MRI in a year. A friend said this is an aggressive cancer and he needs a bone scan and MRI now, chemo and radiation. He is also a Urologist. A relative, who is a reproductive endrocineologist recommended an MRI now and in 6 months, with a PSA and testosterone leval also Two of three recommended that he stay on the Android

I have very carefully considered the case. My suggestion would be to go for aggressive treatment. Let me explain that cancer means uncontrolled division of cells. If there is a single cancerous cell in the body, it is enough to kill the person eventually. If the cancerous cells can be removed before they have spread elsewhere, the chance of survival is 100%. But if the spread had occurred, no matter what you do, its only a matter of time before the unwanted happens. A bone scan will show if any spread has ocurred. It needs to be done at the earliest,. Based on that you would be able to decide if you want to go for treatment and if so, how.

We saw a Radiology Oncologist and he also said Surveillance is a good option, unless the PSA goes higher in 6 months. I feel he should have a Testosterone level now and gradually be weaned off the Testosterone. He is being scheduled for a special MRI, to be used as a baseline. My friend, the urologist, was thinking the XXXXXXX Score was 10 but it was only 6 when he recommended the Bone Scan and Radiation. Do you agree with this option?

PSA right parameter to be monitored. MRI and bone scan suggested.

Thanks for writing back. Things make sense. PSA should be used for monitoring/surveillance and a baseline MRI should be useful. I doubt if testosterone levels need to be measured but no harm in getting it measured. Would suggest a bone scan to be done. It is only following the reports the the decision of radiation therapy can be made.
