I have read through your query in detail.
The MRU (magnetic resonance urography) which is a specific examination of the kidneys, ureter and urinary bladder shows the following

1. The right kidney is made of two parts, named the upper moiety and lower moiety. This is present by birth and the right kidney tissue is separated in to an upper and lower part with two different tubes (ureters) arising from each moiety.

2. The upper moiety shows normal appearing structures but there is a ballooning in the lower moiety. Both the upper and lower moiety are functioning normally but the lower moiety is showing urine collection which was seen earlier as hydronephrosis. The lower moiety ureter is not filling up as expected.

3. The ureters from the upper and lower moiety are connected at one point and below downwards it continues as a single ureter.

4. The lower end of the right ureter is normal with its opening inside the urinary bladder.

5. The left kidney is normal in appearance and has a normal left ureter which opens in to the urinary bladder.

6. There is a suspicious mildly enlarged liver and spleen but this is not a serious concern.

7. Further testing by AGP (antegrade pyelography) or RGP (retrograde pyelography) is suggested to confirm the normal visualization of ureters and any obstruction in the lower moiety ureter. That will help to know any blocks or obstructions.

Thanks for writing back with an update.

It is not a serious or life threatening condition at the moment. However in the long term close follow up is required to take the right treatment decision at the right time.

Being under the care of a good team of doctors is recommended. Right now the kidneys are working fine but the area of blockage in the lower moiety of right kidney is still not well understood. As the child grows and if there is a confirmed area of block then it can be operated and the block can be released. This decision will be taken considering the risks and benefits of the procedure and is more likely to be an elective decision. There is no serious concern as long the kidney function test results are in normal range.
