What Does The TRUS Prostate Ultrasound Report Indicate?

Question: Hi Doc, I am a 61 year old with PSA 8. Doctor did DRE and advised TRUS test. Following are the observations:
- Ms app 16ml in volume
- Few echogenic foci are seen in the parenchyma & calcification
- In the left paremedian lobe, there is seen a hypoechoic well defined nodule ms 1.8*0.9cm
- Regular in outline and capsule is intact.
- Both seminal vesicles are normally visualized.
Please advise and help me understand this.

I have read through your query in detail.
Your TRUS prostate ultrasound report is the following

1. Measures approximately 16 ml in volume.

If they are referring to volume of your prostate gland then anything less than 25 ml is normal.


2. Few echogenic foci are seen in the parenchyma & calcification.

This can be a normal acceptable finding at your age.


3. In the left paremedian lobe, there is seen a hypoechoic well defined nodule ms 1.8*0.9cm.

On the left side of the prostate gland there is a 1.8 x 0.9 cm dark area.


4. Regular in outline and capsule is intact.

The prostate boundaries are intact and the outer capsule is normal.


5. Both seminal vesicles are normally visualized.

This is a normal finding.


With a PSA of 8 ng/ ml, your doctor might suggest a prostate biopsy from the nodule. It is seen that a prostate cancer is present in 9 percent of hypoechoic nodules in biopsy. That means 91 percent chances it will be negative for cancer. However, your doctor will decide if you need a prostate biopsy or not. Some doctors even ask for level of free PSA and also correlate findings with DRE.

PSA can also be high due to infection and in some people it can remain high for years. With current guidelines, it might be important to rule out possibility of cancer in you.
