What Does The Biopsy Report Indicate?

Question: My mother, aged 60, underwent mastectomy for her left breast along with removal of 16 lymph nodes for two tumors less than 2 cm each and 1 cm apart. Her biopsy report revealed pt1 pn0 grade 3 IDC with angiolymphatic invasion and clear deep margins. The PET scan prior to the surgery revealed no metastases. Her tumour is ER/PR- and HER -2 +. Please advice me on further treatment along with chances of recurrence.

Hello mam, thanks for trusting health care magic.
After reading the history it is quite evident that she has high grade fever and that too ER Receptor negative and HER-2 positive which are both bad prognostic factors.
She has chances of recurrance and even need to take radiotherapy because these tumors are not chemosensitive.
You will need a regular basis follow up with the oncologist and should take every sign and symptom seriously.

Thank you Doctor. How effective is Herceptin treatment? Will Radiotherapy be effective for her treatment? Are none of the chemo drugs suited for her tumour in order to be taken along with herceptin?

Radiotherapy is a better and effective option

Detailed Answer:
Herceptin will be a good option but it is very unpredictable that which tumor will respond to it and which one won't but definitely worth a try along with other chemotherapy drugs.
But in general ER negative and that too grade 3 tumors give best response to radiotherapy and also have better quality of life.
First get her evaluated post surgery from your oncologist and start with herceptin and let us see the response.
As of now the margins were clean from tomor so most probably only herceptin treatment will suffice but eventually regular follow up will be mandatory and in case it recurrs then radiotherapy will be the only option.
And in case we give radiotherapy now itself then chances of recurrence will be really less and moreover she is 60 years old right now so will tolerate radiotherapy better right now as compared to 5 years later.
So discuss all the aspects with your oncologist who can examine her and compile the findings with the test reports and this guide you accordingly.
