What Does The Endoscopy Results Indicate?

Question: Dear Sir/Maam

I am 29 years old. My height is 5 feet and my weight is 31 Kg. From past four month i have breathing issues, wherein i feel heaviness in the chest and i feel i am putting effort to breath or i dont feel normal light breathing.
I had my tests done such as CBC, X Ray, PFT and edoscopy. My endoscopy says i am having GERD, gastritis and H Pyroli infection. I have consulted a pulmonologist, he has suggested i have not put enough efforts and so the PFT is not normal. From past two months i am taking domperidone 10 mg 2 times and ILAPRAZOLE tablet ADIZA 10 mg before breakfast and no other medicines.
Improvement is there but there is a very slow recovery and my symptoms are not going completely.
I need advice, should i reconsult another pulmonolist as it can be a respiratory issue and it is not getting diagnosed. and if you feel this is because of GERD then when can this problem be recovered. I am not generally a depressed kind of a person but this issue is worrying me as i feel i can die of choking because of breathing issues. This is effecting my normal life. Kindly suggest what should i do..should i consult a psychologist as well for better results. My reports are attached with the question.

Just checked the reports as well. GERD seems to be severe as by the reports. Do you have vomitings?

I think you need a holistic approach for getting normal in life.
I could think of larynx/bronchospasms because of the GERD and gastritis, sometimes acid which regurgitates back into the throat will try to get into the airway. The larynx would not allow it and thus constrict. So check if the ENT can see any redness over the larynx which indicates my presumption.

If there is no redness, you need to under specific tests for allergic bronchitis and COPD. Your next pulmonologist can discuss this further.

How are your menstrual periods, regular or irregular? You seem to be underweight. Do you try to take very less calories.? Are you married?. You need screening for eating disorders in such a case.

But yes you should consult a psychologist.

I had alergy screen test done (phadia top- done at apollo) which is negative. IgE test normal (reading is 19) and Anti Transglutaminase antibody ( tTG) is normal. X ray chest is normal.

What are COPD tests and other tests that i have to go for??

I am underwt since childhood and find it very very difficult to put on weight. My periods are regular though flow is very less.

Do you forsee a respiratory problem from my case?

I mean to test for asthma or allergic bronchitis. If they are negative then it is very unlikely to be respiratory problem.

My view is about GERD and gastritis as it was severe in endoscopy report. As I said previously check with the ENT if he can pass an laryngoscope and see if the acid reaches larynx and causing spams and less air entering the lungs.

Given other causes are ruled out GERD is most probable cause.

Challenge test

Detailed Answer:
If spirometry is normal or near normal but if there is a strong suspicion the doctor can do a methacholine challenge test / spirometry after giving trial asthma medication. If the test is better after bronchodilators (asthma medicine) then it is positive for asthma.
