Question: anyone can translate the report for me please, any general surgeon?

Hello!

Thank you for the query.

This reports say that you have acute pancreatitis caused by common bile duct (CBD) obstruction. The obstruction has been caused by stones. Elevated GGTP and bilirubin do confirm that.
The CT says that there is only localized in the pancreas necrosis along with the small amount of fluid surrounding the pancreas. This is fine and do not need any action right now.
The ultrasound says that you have stones in gallbladder and dilated CBD (common bile duct).
Usually it is clear that such situation is caused by stones especially that you have some in the gallbladder.

ERCP is the endoscopic procedure where probe is inserted into a bile ducts and stones are removed that way. This procedure should be done during your stay at hospital.
Secondly you should have gallbladder removal. This should be done 2 weeks after the ERCP and acute pancreatitis normalization.

Hi Dr Stanko,

Thank you very much for answered my questions, i just wondering how long do i have to stay in the hospital when the endoscopy and the remove of gallstone done? at the moment I'm on a lot of medication but my tummy still in pain, everytime i eat solid food, it always flared up. and i am sick of fluid diet already, i'm going to see my surgeon next week but at the moment is there anything i can do so my tummy not hurt anymore everytime i eat something? and i noticed on the blood test my liver wasn't good, do i have to worry about that too?

ERCP is extremely important right now.

Detailed Answer:
Endoscopy should be done as soon as you were admitted to the hospital. This would remove the stone from bile ducts and cause free bile and pancreas juice flow. Without that, recovery will take much longer or wont happen ever.
Surgery is not a good idea at this moment. Acute pancreatitis should be healed first.
Pain after every meal is a typical symptom of acute pancreatitis. Patient who has this condition should not take any foods and do not drink for as long as the inflammation is present. Special tube should be inserted through your mouth to small intestine. Special nutrition formula should be given through it.

Your liver tests are fine. GGTP is elevated. This is a very characteristic sign of bile ducts obstruction. Removing the stone from bile ducts should cause liver normalization. This is really not a problem.

So I really do not understand why you are treated in such strange way.

If you do stay at the hospital here is what should be done:
- ERCP (this endoscopic procedure) as the first, extremely important step
- drips and no food for few days
- if the inflammation will get better, you may be given some fluids first to see the reaction
- in the inflammation still present, tube to intestine insertion
- you should also have amylase and urine amylase checked

i went to the same hospital 3 days after i got discharged because i can't tolerate food and drink, it always came out either vomited or open bowel but they only told me i had gastro, so they sent me home the next day after i had couple bags of drips. Do you think i have to go to another hospital or just wait til i see the next surgeon, next Friday? because i can't stand the pain and don't want them to send me home again.

Well, this is very strange.
You have diagnosed acute pancreatitis. You were discharged without proper treatment (ERCP). Your CRP on discharge was 190 (very high). You are unable to eat (this leads to worsening of your general condition). And you du suffer from pain.
Considering this facts, you definitely should be at the hospital right now. What is your stool color? Is it whitish? And urine, is it dark? If so, ERCP is more than necessary.
In your CT there was some necrosis found. This is the source of inflammation right in the middle of abdominal wall. You should have another CT to see if there is progression and if there are any cysts present.

So if you are able to visit another hospital, please do that.

thank you for the quick response,

my stool's colour is brown but my urine little bit blood on it. do you think they are going to transfer my file from this hospital to another hospital because this hospital is very new and already has a lot of problem .

I have no idea. But the reports you have should be enough. This is quite simple diagnosis and nothing else is necessary. Moreover they need to do new tests for sure.
Blood in urine can be due to simple infection. It does not have to be caused by your current issue.
Brown stools color indicates that bile duct is unobstructed. There might be a stone in it, but does not have to cause complete obstruction. It is also possible that the stone has been evacuated by itself. But still ERCP is a standard procedure.

can you recommend me is there anything I can eat or drink for dinner because in Australia is weekends atm no1 will do procedure n I have to wait until Monday and goin to different hospital for different opinion

Any foods will cause the pain.

Detailed Answer:
Your pancreas is swollen. Pancreatic duct is tight. And with every food or drink you take, pancreas tries to produce enzymes. It is a reflex action. Every time pancreas will produce and excrete enzymes you will have pain. So unfortunately no matter what you eat or drink, the pain will appear.
Most neutral for pancreas are low fat foods. So boiled chicken, rice, boiled vegetables, toast can be tried. If its about liquids, water is the best you can try.
