What Does The Following CT Scan Report Suggest?

Question: I had kidney stones in the right ureter 30 years ago, most recent diagnosis from CT scan shows " dilatation of the right pelvic calyceal system and proximal two thirds of right ureter, etiology could not be defined. No obvious ureterolithiasis. Urology consult for possible retrograde pyelogram.

I have extreme pain in the back flank and groin/ovary area. Upon wakening after short periods it feels like a major spasm, deep breathing causes same pain. At what point should I return to hospital? Waiting on renal scan next week. I also have DDD at L4-L5, L5 -S1 sever sciatica on right hip, leg, foot complicating what pain goes with what

To recapitulate:Female/56 - on Percocet - H/O Kidney stones 30 years back - recent CT shows right hydronephrosis and hydroureter upper 2/3rd - etiology could not be defined in spite of CT scan - planning retrograde - pain in back flank, groin, ovary area - increases on deep breathing - feels like spasm upon wakening - renal scan next week - also have sciatica with severe symptoms - wants to know when to return to the hospital...

You may please return to the hospital when the pain is unbearable.

Please discuss one most important point with your Urologist whether ureteroscopy is possible or not, as the direct visualization of the obstructing lesion gives the cause and it may be therapeutic as if the stone is present it can be removed and stent passed. If there is a mass, biopsy can be taken.

This will relieve the pains.
You can have the opinion of a Neurosurgeon about the possibility of further treatment for the sciatica.,
