What Does The Following MRI Report Indicate?

Posted on Thu, 8 Oct 2015
Question: I recently got an MRI done and the report overall is normal & has one observation -
Microvascular ischemic changes in bilateral frontal regions.

One month ago I had severe head aches 2 times which lasted for couple of days each & mostly on right hand side. Since then if I press my right hand side of head near brain it hurts on that moment. I am not sure if the report observation is related to this.
Or could this be related to eyes as my right eye is currently being treated for dryness & started using a specs since last month.

My BP as on last week was 100 / 70
I regularly go to GYM and never have any BP / Sugar issues.

Brief Answer:
Likely migraine

Detailed Answer:
Hi dear, I have gone through your question and understand your concerns.
The MRI finding is incidental and unrelated to the headache. The medications being given to you are of migraine. The feature of unilateral headache is more in favour of migraine. It is unrelated to your eye problem.I will like to know the following information to confirm your diagnosis:
1. whether the headache is associated with phonophobia or photophobia?
2. aggravation of headache by any food item?
In the meantime, you should continue with these medications. They will take at least 3-4 weeks to relieve your symptoms.
Hope you found the answer helpful. Please do get back to me with the information, if feasible.
Wishing you good health.

Hello Doctor - thanks.

1. going back to my report question - so does it mean I should just ignore & not worry?

2. Answering to your question -
1. whether the headache is associated with phonophobia or photophobia? - no none of these. However after I got this dryness issue in my right eye, I can not look direct into any strong lights particularly during driving at night.

2. aggravation of headache by any food item? - no

As I said, I got major headaches only 2 times till date & that too in last one month which lasted for 2 to 3 days- pain on the right side along with vomiting sensation & guidiness. Before that I now realize that I had such symptoms but of very lite intensity last year once in 3 months...but I always used to think that it me be some neck related stiffness etc.

Now the only problem is that I do not have any regular head ache...it is just that when I press my right side of my head or sleep towards right side of my head - a particular portion starts hurting & feels heavy. This symptom is continuously there for last 2 to 3 weeks for now.

Let me know if you need more details.

Brief Answer:
Continue same medications

Detailed Answer:
Hello XXXXXXX
Your symptoms are most possibly due to episodic migraine. You are already taking medications for migraine prophylaxis. You can continue these medication for 3-6 months as required and take pain killers as needed occasionally.
Mri features are non specific and unrelated to headache.
You can get your Vitamin B12 and thyroid checked. Take nutritional food and adequate rest.
Hope you found the answer helpful.

Thanks, I will get the tests done tomorrow.
Just wanted to add that on wed evening while returning home & driving from office...I started getting head ache like feeling after looking into the high intesity lights from opposite side vehicles...I also noticed that I have developed insensitivity to any loud noise / strong lights. So, should I continue these medicines or try any other?

Do you suggest any thing specific in diet? and anything to avoid?

Brief Answer:
Continue same treatment

Detailed Answer:
Hello XXXXXXX
Your headache and symptoms are migraine like.
The medications need to be increased if not relieved after 2-3 weeks as the drug require that much time for complete action.
In the meantime , you can take intermittent pain killers like Naprosyn 500 mg with domperidone combination.

Diet should be taken regularly. Avoid fasting.
Decrease intake of coffee and chocolates.
Hope you found the answer helpful.
