What Does The Following MRI Report Suggest?

Question: Hello Sir,

My fiancee had a very first seizure 20 days ago and it affected on the right part of her body. After checked with doctor he suggested MRI test. In MRI report she has been diagnosed with Neurocysticercosis. There is a larval cyst in left part of her brain which caused edema. Doctor suggested her to take albendazole to kill the larval cyst and some anti epileptic medicine to control seizure and some steroids to control edema. So she started taking this medication.
After second day she again had a seizure and this time her left part of the body affected and caused partial paralysis with facial palsy. Again she visited to her doctor and he increased the dose and also suggested physiotherapy to recover from partial paralysis. Also he said, she has to continue her anti-elliptic medication for 3 years to control seizures. Doctor suggested her for complete rest of 2-3 months with some instruction like sleep 8 hours in a day, do not watch excesive TV, minimum use headphones, travel after putting cottons in the ears, do not go near loud sound DJ.
Actually, we are going to be marry after 2 months. Now I am worried about it. How much chances that she can have seizure again, mainly at wedding time. Should we postpone the marriage?
If I will get marry to her then what are the challenges that I have to face in my life. Will it affect on our married life ? How much time generally it take to recover from this deasese? Is this deasese curable ? I want to clarify all these things because her doctor suggested that not to tell anything about this desease to in laws. Now I am confused about my marriage decision. My parents are also worried.

Hello dear,
I have gone through your question and mri reports and understand your concerns.
Neurocysticercosis is an infestation of cyst of taenia solium in brain.
This is very common in XXXXXXX subcontinent.
The cyst degenerates with time and heals by calcification or disappears.
The location of cyst may lead to varied manifestations and commonly seizures occurs.
Motor weakness may occur transiently for few days after seizure or for few weeks if the lesion and precessional edema is at the motor centre of brain.
In early stage of treatment the risk of seizures are more and even greater if albandazole is given. The steroid helps in reducing edema.

The disease is curable and in most patients (nearly more than 90%) the drugs are stopped after a period of 2-3 years. In few cases the drug may have to be continued or repeated again if seizures are not controlled.
The risk factors for seizure precipitation are drug default, change in drug timing, inadequate sleep, exertion, alcohol intake, simultaneous use of other drugs like oral contraceptive pills and to some extent flashing lights or loud sound (very rarely).
The risk of seizures are more during first 1-2 month of treatment.
The decision regarding postponement of marriage is a family issue. But chances of seizure recurrence may be low at that time if all drugs and precautions are being taken seriously.
The disease will not be having long term effect in your married life and is less dangerous than diabetes or hypertension from which most people suffer.

If your fiancee has truthfully communicated the condition than you should also be welcoming her in your life.
Life is a rough path and anything can happen to anybody at any time , it is the way you choose and decide at the right moment that matters.

Ans : Every drug has some side effects , please name the drug which are being given.

Ans: Yes, it is usually advisable to delay the pregnancy until medications are being taken.
3) Is this disease can happen from mother to baby?
Ans: No , this is not genetically related
3) Can future seizures cause paralysis again ?
Ans: Location near motor cortex is risk for future Todd's palsy . But it's usually very low risk.
4) Can this disease cause epilepsy ?
Ans: In most cases the disease is controlled and

Ans: In most trails and previous guidelines the risk of recurrence is low when drug is continued for 2 -3 years with well controlled seizures.
6) Can we discontinue medication if seizure will not happen for 6 months ?
Ans: In case of neurocysticercosis , if the lesion disappears on MRI after 6 months then some people have tried stopping drugs. The decision varies with doctor.

Hello Sir,

Thank you for providing the informative answers. I really appreciated for your prompt response.

I have just one question left that was skipped from my mind. Can NCC happens again ? I mean is there any more possibilities that new cysts can enter in XXXXXXX and cause this infection again ?

Also, my fiancee had severe headache from last one and half year. Is this because of NCC as she told me this is because of migraine?

Recurrence may be rare

Hello dear,
Neurocysticercosis is through spreading of cyst from intestinal tract and the infection is due to contaminated vegetables or uncooked pork.
In future avoid , uncooked meat, vegetables and salad which are grown underground should be boiled or cooked especially cabagge, radish, carrot.
Chances of recurrence are very low until many cyst are present at the time of initial MRI. Taking albendazole also kills the intestinal cyst and prevent the recurrence.

NCC is usually not characterised by headache and possibility of migraine is more in young female.
The exact diagnosis of headache can only be made by complete history.
