What Does The Following Brain MRI Report Indicate?

Question : I am 40 years male. my brain MRI reveals :- 1. Nodular WHMI in right frontal deep white matter at level of frontal horn and insular cistern...non specific gliosis/?demyelination
2. Mild neurovascular contact of SCA and right Vth Cranial nerve and AICA loop with right VIIth cranial nerve.
3. follow up to see temporal evolution of WHMI after 3-6 months.

I feel little bit tired all day and there is buzzing sound from my ears and pain in knees, pain in left eye and pain in upper jaw with sour taste in mouth always. stress behind the neck.
I have also interavertabral disc herniation L3-4 and straightened cervical lordosis

My neurologist and psychiatrist said nothing to worry but someone advised brain angiography.

please suggest what should i do. how it can be stopped Any medication, meditation or excercises, change in diet will help? i eat lots of fast food fried etc.

My wife expired 2 years ago due to ILD and I m constantly in stress about me and my son.

Hi,

Thank you for posting your query.

I have seen your medical details and also read the MRI reports.

There is no need to worry, as these changes in MRI Brain are common in migraine patients.

You are also depressed. So, both anti-migraine and anti depressant medications should be continued.

You should stop fried and fast foods, and take more of salads and fruits.

Yoga, meditation and jogging/walking would also be very useful.

Angiography or any further tests are not needed.

Thanks for your advice.

My further query is that even after sleeping 8 hours at night and taking tab. modafinil for alertness, I still feel sleepy all day.

If I do some physical activity like 15 min. walk or tennis only I become very much tired and have to sleep again. Every day I have to take pain killer because lower back pain, stiffness in neck and constant frontal face pain.

I m too much tired taking so many medicines but still has to take ( sometimes I feel medicines are not having any effect). I do not feel like normal as I was one year ago.

Even to go for drive 1 hour I do not have strength and I totally avoid flying due to scare that I will pass out. There is so much anxiety in doing anything new. No thoughts of sex also. All my lab tests (lipid, KFT, LFT, etc.) are normal and heart non-invasive angio is also normal.

Please advise how shall I keep my brain active and alert.

Piracetam and Ginkgo biloba may be useful

Your symptoms are related to depression and migraine. No further tests are needed.

You would gradually improve with medications.

In addition, you can add piracetam and ginkgo biloba to increase the brain alertness and concentration.
