Detailed Answer:
Hello!

Welcome on HCM!

I reviewed your uploaded Holter recordings and would explain that the most important findings seem to be cardiac conductance disturbances which mean abnormal slowing of cardiac impulses from the atria to the ventricles.

These conductance abnormalities range from first degree atrioventricular block to second and high degree atrioventricular block.

The reasons for such cardiac conductance abnormalities may be several: degenerative cardiac conductance system changes, an ischemic cardiac disorder, metabolic and systemic inflammatory disorders, certain infectious diseases (Lyme disease, etc.).

Coming to this point, it is necessary to investigate for the presence of the above mentioned potential causes, which may be properly treated.

From the other hand, as Holter monitoring reveals repeated episodes of second and high degree atrioventricular block, and thus the risk of cardiac standstill is high.

In such circumstances, it is indicated implantation of the permanent cardiac pacemaker for preventing the risk of prolonged cardiac asystole and death; maintaining a normal heart rate according to the body daily activity.

You need to discuss with your attending cardiologist on the above mentioned issues.

Hope you will find this answer helpful!

In case of any further uncertainties, feel free to ask me again.

Kind regards,

Dr. Iliri

Why would the Cardiologist who reviewed the test results not advise me of the danger presented? I currently feel good and continue to exercise (running, weight lifting and cycling).

Thanks.

Brief Answer:
My opinion as follows:

Detailed Answer:
Hello again!

I understand the fact that you may feel OK now (or with only a slight symptomatology such as lightheadedness or episodic vertigo, etc.), but you should know that with that evidence of cardiac conductance abnormality (high grade atrio-ventricular block), the risk of experiencing more serious and even life-threatening complications (fainting, syncope, and even cardiac stand still) is very high.

I strongly encourage you to discuss again with your doctor on the opportunity of further medical exploration of the above mentioned causes of such a cardiac disorder and also regarding the indications of pacemaker implantation.

Hope to have been helpful!

Wishing all the best,

Dr. Iliri
