Brief Answer:
Tranexamic acid and Mefenamic acid tablets. USG .

Detailed Answer:
Hi there.
Dr. Purushottam Neurgankar welcomes you to HealthcareMagic.
I have gone through your query and I think I have understood your concern.
I will suggest you the best possible treatment options.

First of all, do not panic.

As per your history, I will suggest you to urgently seek prescription help from your treating gynecologist for Tranexamic acid and Mefenamic acid tablets. This will help to control the bleeding.

I would like to know if you have done USG in the last 3 months?
If not, I will suggest you that in the next few days, please get USG done to know about the structural changes in the uterus, if any.

I hope this answer helps you. Feel free to ask further.

Thanks.

Thank you for response...I would like to add that I had stated above that I recently had my annual pap smear (5 months ago) and it was completely clear from displacia and hpv...but....prior to this pap smear I had 3 bad paps over the past 3 yrs that indicated HSIL...but was never tested for hpv with those paps...I never had treatment because I didnt have insurance. But like I said when I had my last one done 5 months ago everything was gone...they retested to make sure and it was all clear. Do you know how or why HSIL would just go away? And could this really heavy period (which has ended now) mean that things are going bad again? Thank you for the time you have put into my query...it is very much appreciated.

Brief Answer:
PAP smears at regular intervals. Safe and protected sex.

Detailed Answer:
Hi there,
Thanks for appreciating the answer and reverting back.

It is good to hear that your PAP smear reports have got normal.
HSIL_ High grade Squamous Intra epithelial Lesion.
Majority of the mild and moderate SIL are reversible.
HSIL needs meticulous follow up with PAP smear at regular intervals.
As cervical lining has repaired and healed in your case, the PAP smear reports might have been normal.

I will suggest you to get PAP smears done regularly.
Please observe proper vaginal hygiene. Use of Lactic acid containing wash at bath time is advisable.
Please seek proper guidance about safe and protected sex activity.

DO REMEMBER, PREVENTION IS ALWAYS BETTER THAN CURE.

I hope this answer helps you.

I'm sorry...I dont want to keep asking follow up questions because I dont know if that is allowed..But you sound as if my clear pap done recently may not have actually been clear because HSIL doesnt repair itself? Am I understanding that correctly? I would like to add that I have been married to the same man for 10 yrs now. Help me to understand HPV please...I know that is the leading cause of Cervical Cancer and to get Cervical Cancer without being positive for HPV would be pretty rare correct? Does HPV go dormant in the body like Herpes? Or do you catch again from changing sex partners? Or does it come back if your immune system gets run down? Please share any experience with this as I am trying so hard to understand so I can care for my self properly until I can get in with an obgyn. And thank you so much for your patience.

Also to add I did have an ultrasound 4 months ago to check for abnormalities and all they found was two tiny fibroids that they said where not in a location or big enough to cause any real issues with my periods, but I dont know enough about fibroids to know if that information is correct or not. But they said they were very tiny and a non issue.

Brief Answer:
don't worry about fibroids. Colposcopy is advisable.

Detailed Answer:
Hi there,
Thanks for appreciating the answer and reverting back.

I will go to your second query to start with,
Small, tiny fibroids, that are not causing any symptoms, can be left alone. You need not worry about them. Just a regular USG at 6 monthly interval is advisable.
About 65 to 80% women have some fibroids, it is the location, size of the fibroids and symptoms that needs consideration.

Secondly, about your concern about HPV.
All your concerns and questions are the answers for themselves.
HPV- human papilloma virus , can get transmitted with or without history of change of the partners. It depends on your hygienic habits, immunity that a particular strain of HPV gets active.
HPV is one of the cause of cervical cancer, and the only cause.
It makes cervical lining more vulnerable to pathological changes.

For your HSIL, i will suggest you to get colposcopy guided cervical biopsy, this will help to confirm or rule out HSIL.
You are right in having your information that HSIL does not get corrected on its own. So before taking any further action, please get colposcopy done.

I hope this answer helps you.
Thanks.
