Question : I had vasectomy twenty years back but recent analysis of semen shows 25 million, 10% active, 40% sluggish, 35% non motile, 05% headless, 05% tailless, Do I need to have vasectomy again or the earlier one can still prevent pregnancy. I don't need a child

Your vasectomy seems to have failed.

However another possibility is that semen analysis report is wrong.

Please repeat semen analysis in another standard laboratory after explaining them your vasectomy condition.

If report again shows sperms you should consider this as vasectomy failure and repeat the procedure.
