What Does The Following Semen Analysis Report Suggest?

Question : Hi sir ,

My semen analysis shows : Abstinence period : 3 ,
physical examinations : quantity : 1.0 ML , colour : whitish , reaction : Alkaline , viscosity : Normal
Microscopic Examinations : Total Sperm Count : 24 millions / ml, normal range (60-150 millions/ml).
Motility : Active Motile : 20% , sluggish motile : 15% , non motile : 65%
Morphology : Normal : 28% , abnormal : 72%

PUS cells : 6-7 /H.P.F , EPI, CELSS : 3-4/H.P.F, RBC CELLS : Absent.

Hello Mr. XXXXX,
Thanks for writing in to us at hcm.
This is Dr.Riyanka and I am glad to be helping you with your query.

Based on the semen Analysis report there are certain points to be noted:

1. Quantity is 1ml. Volume of less than 2 ml should be investigated for any causes of obstructive azoospermia i.e; there coud be an obstruction in the semina vesicles or could be due to insufficient sample.

2. Motility: Alteast 50 % sperms are expected to be normal and motile for good fertility.

3. There is a little confusion in your report as it is not stating clearly between total sperm count and sperm concentration per ml.
Total sperm count of less than 40 million(i.e sperm concentration/ml X Volume of semen ) is taken as abnormal.
Where as Sperm concentration should be atleast 20 million/ml to be called as normal. Therefore the reading of Total sperm count of 24million/ml is not accurate as Total sperm count gives the number of sperms present in the entire sample and cannot be expressed in per ml.

4. Morphology: Atleast 30% sperms should be of normal morphology.

5. Presence of 6-7 pus cells/High power field shows infection. Hence needs to be treated with broad spectrum antibiotics like Ciprofloxacin Tinidazole combination twice a day for 10 days, provided you are not allergic to these antibiotics.

The semen Analysis is definitely not normal as expected . However, donot worry stay calm.
I strongly recommend that kindly consult an XXXXXXX regarding the medications and further investigations like:
1. Sperm Chromatin Structure Assay (SCSA)
2. DNA Fragmentation Test (Reprosource)
as these are more informative regarding the DNA of sperms.
