What Does The Following Test Report Indicate?

Question: Hello Dr,

I had a quick chat with you few days back and you have suggested me to few tests, I had those reports with me. I will upload the same into the system Please look into it and let me know.

I have assumed the question will go to the Dr which I had a conversation, I am giving the conversation details below. Thanks for the help.

Doctor: I am Dr Vaishalee. Please proceed with your query.
User:Hello Dr...10 days back I had a chat with one of the Dr with the below symptoms...
User:1. Feeling very week and dull. 2. Dizziness and sweating after a little physical exercise (even for few mins of walking.) 3. Light-headedness. 4. A feeling that my muscles got weakened. 5. Mild knee and hip pains (but not continuous..once in a while) 6. If I hold any thing with my hand...wrist pain starts. 7. Lot of hunger, within couple of hours of food i feel hungry. 8. One more observation is that I became little thin since 6 monthsUser:He suggested me to wait for few days then visit a Dr.
User:couple of days back I have visited a Dr...and he suggested me to get B12 report..
Doctor: ok
User:I got the report today... and the count is abnormal...
Doctor: ok
User:which is greater than 2000
User:My Dr said it's not an issue...But I heard from my friends having more is not goodUser:I wanted to know is it something I should worry or ignore

User:yes..they are mild.....weakness is the main problem to me...
User:dizziness..is not continuous..but it has..I am able to manage

User:Actually a couple of months back I had a complete body check up
User:Almost all results are pretty fine
Doctor: that includes fasting blood sugar, complete blood count, liver fuction testUser:yes

User:before also i have those symptoms....i got a doubt that it could be a diabetic problem....hence went for complete body check

User:do i need to worry for anything here?
Doctor: I would repeat the vit B12 test and the other three that are mentioned

Seems like erythrocytosis

Detailed Answer:
Hi and thanks for your query.

I have seen the reports. The vitamin B12 levels are raised. So is RBC count and rdw. The other cell like that is leukocytes and platelets are normal in number. Spleen is normal in size and kidney and liver are also normal of ultrasound. This report does rule out polycythemia (essentially polycythemia vera). I am thinking that it is erythrocytosis along with elevated vitamin B12 levels.

I need to know if you have plethora that is redness of face. Also do you take alcohol or smoke cigarette (passive or active)? Do you have dehydration from any reason? Do you have any lung congestion?

Since you have dizziness, I would suggest that you see a neurologist to check for cerebellar signs and appropriate tests. A chest Xray may also be done. Erythropeitin levels may also be raised and can be checked. You may need phlebotomy that is shedding of blood to decrease the viscosity of blood and reduce the symptoms.

Erythrocystosis mostly happens due to cigarette smoking.

A hematologist may also be consulted.

All this will help in pinpointing to the cause of these findings. It does not seem ominous though.
