Question : 2 and a half weeks ago, I had a urine test and my RBC was 3-10. WBC was 0-5 and mucus threads were present, with no bacteria seen. My Occult blood was 1+ /Abnormal. Prostrate Specific Ag, Serum (Roche ECLIA methodology) was 1.7. Now I’m having shooting pains in scrotum about once a day, or they shoot up into my stomach too. No odd colored urine, or burning or need to urinate frequently. I’m 46, 6feet tall, weight about 180 pounds, have never smoked, drink, or done drugs. Been celibate 2+ years.

I'm also XXXXXXX Dunno if that matters. Have never had any major illness with myself nor with my family on the male side, far as I know.

How much longer must I wait? This is taking too long?

Brief Answer:
Tests are normal, just indicating inflammation of urinary pathways...

Detailed Answer:
Hello my dear XXXX,

I have just received your e-mail and I'm here to help.

I read your message carefully and understood your problem.

The urine test can be considered normal although, small traces of WBC, mucus, and occult blood may indicate some inflammation there.

PSA (Prostate-Specific Ag) of 1.7 ng/ml in a man 46 years old is normal.

I am glad to see that your are generally healthy. But you need to see a doctor who will do a physical examination and determine the cause. Further investigations may be needed like an abdominal ultrasound for example to rule out stones in your urinary tract or kidney or any obstacles in your urinary tract.

Do you notice any lump in your groin area? An inguinal hernia can give exately the symptoms you experience: pain is shooting, it radiates to lower abdomen and stomach and goes away when you usually lie down. If pain does not go way it is an emergency - you need to go to doctor fast.

Your upper urinary tract must be seen as well. Any inflammation near the uretheres may cause the same symptoms. Examination of testicles and tests are required too.
If you are not circumcised, and having difficulty in emptying your bladder, then, should consult with urologist and think over circumcision as it will clear off your symptoms.

Hope this is helpful. If you have other questions feel free to ask.

Do I need to see a specialist like a urologist, who can do all these things - the physical, the abdominal ultrasound, examine my scrotum & whatever other tests or I can stay with a doctor who does internal medicine? I can't afford to be seeing doctors all over the place who might give me vague assessments then I still end up having to see a specialist when I'd have done that initially? I had visited a Community Health Center for my annual physical & when I asked them about the things in my urine, not understanding what it all meant, which I stated to you initially, the doctor brushed it off saying it was of no consequence but the nurse later told me as I was leaving, that it'd indicate an infection, & my instincts told me to trust her. From what I told you can you definitely rule out a UTI, or it's more likely to be what you say? Also can you list how much everything I need to rule out might cost me in Northern New XXXXXXX since I'm uninsured? When I take my tumeric either in pill form, or as food additive it feels as if it goes away for the day? I've not noticed any lump in my groin but I seem to have minor sores underneath or to the side of my scrotum, which aren't exactly sore? I don't want to end up being rushed to the emergency room, since even with a urologist it may take 2 weeks to schedule the nearest appointment & we're headed for the holiday season so many doctors may schedule sparing. Gimme something more definite to go on, & a quicker response.

The minor sores underneath/to side of scrotum are just 2 minute ones, no pus comes from them or any odors. Also what does "inflammation of urinary pathways" point to - an UTI? I'm not having any problems emptying my bladder, nor has its normal frequency increased.

Brief Answer:
Nothing to worry is noted, advised as following...

Detailed Answer:
Hi back,

Thank you for providing additional information although I wanted to have a look at your test reports to evaluate your case better.

Once again, from the tests reports you are providing, I can judge that there is an inflammation of your urinary tract. I cannot admit it is an UTI as long as I do not have the other parameters to analyze.

You seem to be a bit worried, and you shouldn't as long as no symptoms you are having. For the moment, I can only tell to :
- increase fluids/plain water intake
- take urine antiseptics (containing uva-ursi or cranberry) that you may find over the counter for at least 1 month from now.
- take over the counter analgesic (ibuprofen or mephenamic acid) to ease the soreness

After one month, you can ask for another urine analysis or can buy one at home urine test and can repeat urine analysis to see the changes there.

If still the same scenario, you can ask to consult further an urologist. Your case is not an emergency, so can wait until the appointment is set. But before, go for the changes I advised earlier.

Thank you.
Dr.Albana
