What Does The Following Widal Test Report Indicate?

Question: I have done my Vidal test today and report is as follows . Kindly suggest is this typhoid.

TO 1:320
TH. 1:160
P.AH 1:80
P.BH 1:40

I am suffering from fever from last days along with headache . Kindly suggest

Widal test & Typhoid fever

Hi,

Thank you for your query. I can understand your concerns.

Your Widal test TO 1:320 & TH 1:160 is suggestive of typhoid fever considering you are also symptomatic with fever for few days.
The Widal test is positive if TO antigen titer is more than 1:160 in an active infection.
However rising titre as opposed to single titre has more predictive value and rules out false positivity.
The most confirmative test of the Typhoid fever is positive culture (blood,urine,bone marrow aspirate) of S. Typhi.
However percentage of positive culture (the sensitivity of blood culture
is only 40–80%), is low due to prior administration of antibiotic.
In view of this it is justified to start antibiotic therapy for typhoid fever in your case.

Empirical antibiotic therapy for Typhoid Fever in Adults:
Cefixime 400 mg bid orally for 10–14 days.
or
Azithromycin 1 g/d orally for 5 days.
alternative regime Azithromycin 1 g on day 1 followed by 500 mg/d orally for 6 days.
Consult your doctor as these are prescription drugs.
