What Does The Semen Analysis Report Indicate?

Question: Gynac:

I have done semen analysis last month.

Results: Grade a: 0%, Grade b: 60%, Grade c & d: 40%

Can i still have a baby, since grade a is 0%/. what are chances of conception with grade b and how many hours before you should have contact.
My wife's report are normal..we are trying from 1.5 year but no success.Her all tests are normal

Absence of Grade a motility means that progressive motility, which is required for successful fertilization of the ovum, is absent. This can mean that chances of natural conception are less likely.

Though Grade b motility is good, these sperms tend to move in a disorderly fashion, though forward, and hence cannot be relied upon to cause fertilization.

Normal sperms can live in the female genital tract actively up to 3-5 days in general, while the ovum has a life of only 24 hours after release. So, in order to cause conception, sexual contact should be made preferably within a few hours of ovulation, with the grade b sperm motility.

I have gone through your report. Most of the findings are normal. Forward motility should be at least 50% after one hour. Sample obtained through condom contains higher counts and hence is considered better than the one obtained through masturbation.

I would suggest waiting for another few cycles and also to repeat the test after three months and meanwhile, try to quit caffeine, smoking, alcohol etc., if you are in the habit.

There is no harm going for IUI. It actually helps when sperm count is lesser and motility is low.

Finally, even if natural methods fail, you still can go for IVF as your counts are good.

can you tell me looking at report how much is grade a percentage? I can see it as 0 because the report shows as grade b....how can you say its normal report?

I dont smoke,drink...
Additionally which fruits,vegetables should i eat more? I start fortex capsule last 15 days to increase motility.

The grading used in the report is perhaps the 0-4 grading and not the a,b,,c. In this grading of motility, 0 means absent or no motility, while 3-4 indicate good motility. The report indicates that the sperm motility is good.

Fortex is an ayurvedic preparation meant for general well being. You can eat dry fruits like almonds, fruits and foods rich in vitamin C, E, and zinc, folate.

Going by this report, you can safely try for natural conception for another six months and get further help later if you do not see any result.

You mentioned before that you should have 'a' (=grade 4) for effective. Can you please re-open my lab report and see that they have written as 65%(grade 3-4) and on top of that line they have written grade 3.

I think grade 4 is absent in my report.Can you tell me my grade 4 percentile from my report.

I hope you have seen the normal values for the lab assigned separately for the percentage and grade. While percentage indicates the motility after 1,2 and 3 hours, grade indicates the linear progressive motility which is the main feature necessary for conception. According to this, your grades are within normal limits.

Grade 4 has not been mentioned in your report and please check with the lab what criteria they follow to assign grade 4, since they have not mentioned it in the comparable normal ranges either.

I checked grade 4 is very rapid moving.
Very rarely found in males mostly after washing sperms. Am I right?

You mean to say report is normal? We have been trying for almost 18 months... In middle due to stress I had low motility.

Is it unexplained infertility .?? I am getting frustrated with my age also.

We normally try in doggy style sex... Do u suggest other position

After washing of the sperm, the prostaglandins are removed and the concentration improves. This procedure is adopted for IUI purposes and ART procedures.

Your semenalysis is apparently normal. If you have been sincerely trying for so long, you should definitely go for assistance now.

Unexplained infertility should be diagnosed only when all investigations come out normal and yet there is failure to conceive.

You need to maintain patience as treatment for infertility is a matter of slow progress many-a-time and perseverance is a must. You need not worry about your age as you are still young.

Missionary position is the most common and useful position. Also, after intercourse, your wife should lie still for about 30 minutes, should not wash her soft parts with strong soap solutions. If she has an anteverted uterus as seen in the sonogram, she should lie on her back; if the uterus is retroverted, she should lie on her stomach. This will help easier transit of the sperms.

I mentioned the style followed by the majority and if you have difficulty with it, you can choose one from the several methods available and you can browse them on internet. There is no hard and fast rule as to which is better and it is a matter of individual convenience.

Sexual intercourse should be done preferably daily or at least on alternate days around ovulation.

Post coital test or PCT is preferably done 2 hours after intercourse and is used to test for any anti-sperm antibodies and compatibility of the sperm with cervical mucus. You cannot get any reliable result now after so many hours.

It is true that fertilization occurs within few hours of transit if ovulation occurs at the time of the intercourse and implantation occurs within a week. Estimation of the serum beta-hCG titres can be done about 7-10 days after the fertilization to confirm pregnancy. To do a home pregnancy test, one should wait for the next natural period and go for the test 3-7 days later if the period is missed.
