I had a cat scan on 9/13/13 and another one on 6/10/13. the reason for the cat scan was just precautionary because I was a heavy smoker

Brief Answer:
Biopsy

Detailed Answer:
Hi
XXXXXX
Since your CAT scan is showing a spot in your left lower lobe of lung and since you are a heavy smoker i would suggest a CT guided biopsy from the space occupying lesion.
Diseases such as tuberculosis and pneumonia can also cause a spot in your lung.
So i would suggest a biopsy which is a procedure to remove a piece of tissue or a sample of cells from your body so that it can be analyzed in a laboratory. If you're experiencing certain signs and symptoms or if your doctor has identified an area of concern, you may undergo a biopsy to determine whether you have cancer or some other condition.
While imaging tests, such as X-rays, CAT scan are helpful in detecting masses or areas of abnormality, they alone can't differentiate cancerous cells from noncancerous cells. For the majority of cancers, the only way to make a definitive diagnosis is to perform a biopsy to collect cells for closer examination.
Hope i have answered your query. If u have no more clarifications then please close the thread.

DR De

I had a cat scan on 9/3/13 and everything was fine. on 6/4/14 it showed a 2cn ununiformed lesion in my lower left lung. On 1/14 I developed a every cough mainly every night in Palm XXXXXXX and when I moved back to XXXXXXX on 5/15 the cough reduced to less than 10% of the time. I am no longer a smoker for 2 years. also, on1/15 I cheated and smoked for 1 month an for 2 days I spit up blood so I stopped smoking and the blood disappeared completely. 2 questions. 1) is it likely I could have pneumonia and would that show a lesion? 2) if cancer, would it be caught early enough to have a cure or is it possible to be stage 4.
I will try again. cat scan info. lung window images: mild interstitial density is seen in the posterior right apex which is stable. mild diffuse generalized increased density is present throughout right lung. near the right cardiophrenk angle the right middle lobe shows branching linear areas of density which extend between the diaphragmatic and chest wall pleura to the pericardium and appear stable. Stable mild interstitial pleural density is also present in the medial right lower lobe. the left lung apex show a minimal posterior pleural irregularity. the medial basal segment of the left lower lobe show a peribronchial thickening., increased interstitial density, and reticulonodular change, associated with loss of volumn. this results in a plaque-like mass measuring 1.7 x 5.1 cm seen last on 9/3/13. the left hilum on 9/3/13 showed a 0.6 x1.5 cm semilunar calcification in close approximation to the lower left lobe pulmonary artery. a soft tissue mass has developed in the interval around this calcification measuring 1.8 x 2.5 cm which nis thought to represent an enlarged left hilar lymph node . the aortopulmonary window nodes are not enlarged; subcarinal lymph nodes appear stable but mildly prominent. please advise your opinion of what you think!! I took my last cat scan 6/10/14 which is 9 months from 9/3/13. XXXXXXX

Brief Answer:
Lung cancer staging

Detailed Answer:
Hi
XXXXXX
You do not need to worry at this moment.
Firstly sometimes pneumonia can show as a lesion in CAT scan.So i would advise you 1 week of course of antibiotic with amoxicillin and clavulanate. After that repeat a CAT scan and if the lesion is still found then do biopsy from that lesion to rule out malignancy.
Secondly for staging of lung cancer u need to do further tests such as CT scan of upper abdomen and Bone scan.If the lesion is found to have spread to liver, adrenals or bones then it is straight away stage 4 lung cancer.
If the cancer has not spread to above mentioned areas then it is stage 1 or 2 and surgery with chemotherapy after surgery called adjuvant chemotherapy can cause cure of most patients but if it is found in stage 4 then palliative chemotherapy is the only option.

I will try an shorten the question as I am not getting any answers; from my PET scan , the final said IMPRESSION : I take to mean summary. anyway it states 1) left hilar and infrahilar masses, most likely lymph node metastatic disease. the primary tumor may be the area of consolidation in the medial left lower lobe or may be contiguous with one of the hilar metastases. 2) no evidence of distant metastatic disease. do you think I caught it early ? does it require major surgery? my right lung was clear.

Brief Answer:
Early stage

Detailed Answer:
Hi
As there is no distant metastases in PET CT scan u may be in stage 1 or 2 cancer.
So in your case surgery is the option as u seem to catch the disease in early stage.
As it is in early stage surgery and radiation are options for you.
Curative surgery is a good option for u.
Wish u good health.

Dr De; thank you for you response. I need to ask you a couple more questions. from the Pet scan the last heading was IMPRESSION. does that mean summary? there was info on the report with more detail as to size. do you need that info? it said there is no abnormal FDG activity in the abdomen or pelvis. Physiologic activity is seen in the kidneys, ureters, bladder, and bowel. does that mean they are functioning ok? when I get done here I will give your review which so far you have been a great help and will show my appreciation when done. I am prepared for good and bad news. thank you again

Brief Answer:
Thoracic surgeon

Detailed Answer:
Hi
In your case whatever the size of the tumour it can be removed by surgery as it is in early stage and there is no distant metastases.
No abnormal FDG activity in the abdomen and pelvis means there is no increased metabolic activity in these areas which can be caused due to the spreading of tumour from lungs.
Physiologic activity seen in kidneys, ureters, bladder and bowel means this organs are functioning absolutely fine.
So u do not need to worry and u need to consult a thoracic surgeon for surgery as early as possible for curative intent.
Wish u good health.
