What Does This CBC Test Report Indicate?

Question: Dr. XXXXXXX answer quite a few questions yesterday on my CBC from July. He did not think any signs of a blood cancer, but I did get new results today. I have lost 60 pounds in 3 months on purpose. I was not losing weight until I was told my spleen is enlarged at 14.5 cm on ultra sound. Since then I have changed diet, exercised, and just being scared has brought the weight down. A ct scan says the spleen and all other organs normal. So a little conflict and I think I can feel the spleen, so I think it might be enlarged even though the ct said normal limits. with the new results all numbers are in the normal range but have dropped. Would losing the weight cause this, or is this a sign the spleen filtering out the cells causing the enlargement? Just concerned. This is a CBC with diff and no indication or flagging of abnormal cells. Is this drop normal because I have lost weight? Just concerned because I am sure I can feel my spleen under my ribs when I breath. I know over time an enlarged spleen can drop the numbers. Just want a doctors opinion on if this drop can be normal or do these numbers usually stay the same. is the drop in numbers a concern? Thank you.

I am Dr. Jyothi BL and I will be answering your query.

Looking at the reports and current query, my opinion is as follows:

1. Even though it appears to be mild reduction in most of the values, they are still within normal limits.

2. Spleen gets enlarged in various hematological conditions as well as infections. Good to know, you have lost 60 pounds. Overweight and obesity can be inflammatory conditions, which could have caused splenomegaly. As all the hematological values were within normal limits, no hematological cause of splenomegaly could be seen.

3. CT scan is very useful to assess spleen size. Yes, when we take a deep breathe, spleen moves down. Normally when we say splenomegaly, it should be easily palpable and we will be looking for a cause.

4. All the pre and post values are normal. Try to maintain a nutritional diet with moderate exercises.

The drop in numbers is not a concern. You can get evaluated after a month or two and if it is further reducing, which i would not expect to, if you maintain a nutritional diet, then we can think.
Until it is within the normal range, not to worry.
Dirunal variation, day to day variation and physiological and pathological causes will always change blood values. If its within normal limits, never worry.
