Question: Hi Doctor,
I am at my 34th week of pregnancy, and so far all the reports and scans have been normal, however I have developed a bit of itching in my feet/ sole(not very severe). I am more worried about this as I already have gallstones from the past 7 yrs, however, its mostly passive stones and have not been suffering because of that. But I am worried if there is cholestasis of pregnancy. I also underwent a Liver function test recently and the result is as follows:
Test Name Test Method
Biological
Result Reference Interval
LIVER FUNCTION TEST (Serum)
TOTAL PROTEIN 5.7 g/dl 6.0 - 8.2 g/dL Biuret method
ALBUMIN, SERUM 2.62 g/dl 3.5 - 5.2 g/dl BCG, End point
GLOBULIN 3.1 g/dl 2.5 - 3.5 g/dl Calculated
A/G RATIO 0.9 1.1 - 2.5:1 Calculated
BILIRUBIN TOTAL, DIRECT AND INDIRECT
BILIRUBIN TOTAL 0.27 mg/dl 0.3 - 1.2 mg/dl Timed end point method
BILIRUBIN DIRECT 0.06 mg/dl 0.0 - 0.3 Timed end point method
BILIRUBIN INDIRECT 0.2 mg/dl 0.3 - 1.2 mg/dl Calculated
SGOT 23 U/L < 35 U/L IFCC
Test Name Test Method
Biological
Result Reference Interval
SGPT 14 U/L < 35 U/L IFCC
ALKALINE PHOSPHATASE, ALP 95 U/L 42 - 98 U/ L Enzymetic
GAMMA GT (GGT) 14 U/L < 38 U/L Enzymetic
Most of the values are slightly less than normal for me

Let me know if I need to do any further tests, or is there anything to worry about, also my fetal movements are quite good, with progressive increase of movements week by week.

Brief Answer:
Cholestasis of pregnancy not at present

Detailed Answer:
Hi dear,
I have gone through your question and understand your concerns.
I have reviewed your reports.
The LFT is completely normal and is not positive for cholestasis of pregnancy.
Slight itching in the body can be due to pruritis of pregnancy, which is a physiological phenomenon due to hormonal changes.
However, cholestasis of pregnancy can be confirmed by bile salt levels, which most of the labs don't do.
So, I will suggest you to repeat the LFT after 2 weeks to see any change in LFT levels and detect the cholestasis at the earliest, if the itching persists.
In the meantime, you keep a strict count of fetal movements and take adequate rest.
Hope you found the answer helpful.

Also is there anything to worry about

ALKALINE PHOSPHATASE, ALP 95 U/L 42 - 98 U/ L Enzymetic

the level is in the upper limit of normal, is it common during pregnancy, or could it be the level raising slowly, and should be alarming?

Thank you

Brief Answer:
Its normal

Detailed Answer:
Hi dear,
Alkaline phosphatase is normally at higher level of normal range in pregnancy because the placenta releases the alkaline phosphatase in blood, which increases its level.
There is absolutely nothing to worry.
