Question: I had an mri done last wek due to back pain and buttock pain and tightening down the front of my right leg.. It says I have 1mm anterior listhesis of L4 upon L5 with mild circumferential disc bulging. and mulitlevel thoracolumbar degenerative disc disease most pronounced at L2--L# and L4-L5 Of incidental note L2 and L3 have vertebral body hemangiomata.. Hypertrohic degenerative changes are noted, exxagerated through the mid/lower lumbar spine facet joints with mld/moderate lumbar facet join arthropathy. hypertrophy of the ligamenta flava throughout the mid/lower spine is observed as well.Osteophytic spurring involves are lumbar vertebral bdoies to varying degrees, the T11-T 12 disc space demonstrates developing disc desiccation/dgeneration with moderate circumferential disc bulging, without signficant central canalicular stenosis and /or foraminal stenosis.L2-L3 shows disc dessication /degeneration with moderates space narrowing with circumferential disc protusion .When considered with the degenerative changes mild/moderate right sided foraminal stenosis is observed with mild left sided stenosis with moderate central caniclular stenosis. The l4-L5 demonstrates disc desiccation/degeneration with mild circumferential disc bulging. Mild central analicular stenosis is observed without significant foraminal stenosis. L4-L5 intervertebral disc space demsonstrates disc desiccation degeneration with 1 mm anterior listthesis of L4 upon L5 with mild circumfernetial disc bulging. Hupertophic degenerative changes (exaggerated thorught mid/lower lumbar spine facet jponts with multilevel thoracolumbar degenerative disc disease most pronounced at L2- L3 and L4-L5 .. This all sounds scary to me and what is the prognosis outcome .Will surgery become necessary or can it be treated otherwise?

I have gone through your query and understand your concern. You are having degenerative spine disease with mild spondylolisthesis. It is common in your age. Its initial treatment is by medicines (conservative) and back strengthening exercises. Glucosamine and Mecobalamin is helpful in treatment. You can take analgesic such as ibuprofen for pain relief. If symptoms are not reduced by medicines you may require operation (decompression). Treatment options depends upon your symptoms
