Microangiopathy means disease of small blood vessels. Various conditions like diabetes, Hypertension etc can affect blood vessels resulting in making them thicker, weaker, leakier and sluggish blood flow which in turn finally result in damage to the organ being supplied by the blood vessels.

I suppose that the report is for MRI scan of the brain. The MRI report indicates microangiopathy (disease of the small blood vessel) resulting in reduced volume of brain parenchyma. This can manifest clinically as reduced cognitive function of the brain.
