Question: Hello sir,
FINDING of my TMT report is as under.
"Patient could exercise for 06 minutes and 44 seconds of the XXXXXXX Protocol and achieved a workload of 8.50 mets. He attained a peak heart rate of 155 beats/minute which is 96% of the predicted maximum. The exercise was terminated owing to attainment of THR. There was no classical angina. Clinically the blood pressure response shows mild increase from 120/80 to 150/80 and there was no S3 and S4 gallop in the recovery period. In the pre exercise there was no significant ST segment change. At peak exercise there was 1.6 -2.5mm ST segment depression in leads 2,3aVF and 0.5 to 1.4 mm in leads V5-V6. Increased ST segment response in leads 2, 3aVF and in leads V5-V6 appeared in the 6th minute of the exercise at the heart rate of 140bpm and lasted for <1 minute into the recovery period."Please let me know meaning of above mentioned result and impression of TMT test? I am 59 years old person.

Brief Answer:
Exercise-induced ischemia-Possible CAD.Need CAG.

Detailed Answer:
Hello, Sir.

I understand your concern.

I have gone through your query.

All the parameters pertaining to heart rate, blood pressure and work done by heart are within the normal limits.

Pertaining to ST changes, this needs a concern.

The ST depression noted implies exercise-induced myocardial ischemia(which means reduced blood supply ) to heart.

Even though they have lasted less than, it needs further evaluation through a coronary angiogram/CAG to identify any blocks in the coronary blood vessels.

A normal CAG would only be helpful in ruling out significant coronary artery disease-CAD.

Post your further queries if any,

Thank you.
