
What Does This Biopsy Report Indicate?



recently had discomfort in digestion, severe bloating, gas formation, frequent sensation of motions
went to gastroenterologist, he made an endoscopy and endoscopic biopsy
The biopsy result is as below, I'm scared now, please explain the situation below and suggest a solution
Stomach (endoscopic) biopsy

NATURE OF SPECIMEN
Received single grey white mucosal tissue bit
measuring 0.3cm. Entire tissue processed.

MACROSCOPIC EXAMINATION
Section shows single small gastric mucosal bit
with superficial erosion of surface epithelium in
focal area. Foveolae and glands are
unremarkable. Lamina propria shows moderately
dense diffuse infiltrate of lymphocytes, few
plasma cells and a rare polymorph. Lymphoid
follicle formation is seen in one focus. Scant
muscularis mucosae included shows no significant
pathology. No definite H pylori, intestinal
metaplasia, glandular atrophy or dysplasia or
malignancy noted in the section studied.

MICROSCOPIC EXAMINATION
Moderately dense diffuse chronic inflammation -
Stomach (endoscopic) biopsy.
Nothing to worry, however routine and timely follow-up to continue..

I've gone through your query and understand your concern..
Yes, regardless of those symptoms, an endoscopy followed by biopsy is definitely terrifying for any one, specially when at such young age..
However scary looking those images may look like, it's not really scary. However if you look close closely, those flat looking multiple white spots are those scattered ulcers through out your stomach mucosa, surrounding a lot of inflammation, hence the final impression was made as Diffuse erosive Pan-gastritis..
Good news is RUT and H.pylori is negative, else it would be quite a bothersome problem..
It's nothing serious, and perfectly curable condition over a period of time(duration depends from person to person, overall drug compliance and life style modification and maintenance)..
So here's what you should do :
1. Tab ESOMEPRAZOLE/DOMPERIDONE(40/30 mg) Once Daily in empty stomach (preferably before brushing) 20-30 min before breakfast for 1 week(available by the name of NEXPRO-D/NEKSIUM D), followed by Tab ESOMEPRAZOLE 40 mg Once Daily in empty stomach (preferably before brushing) 20-30 min before breakfast for next 3 weeks..
2. Stop smoking and/ cut down alcohol intake(if you do)
3. Avoid junk food totally, no spicy/deep fried/out-side restaurant foods
Take simple home cooked meals
4. Have yogurt on daily basis(unflavored preferably) with or after meals (breakfast/lunch)
5. Include whole grain products, green leafy vegetables in your diet
6. Avoid fizzy/carbonated drinks as well as packaged juice, have fresh fruit juice
Avoid too many tea/coffee, don't drink tea/coffee in empty stomach, 1- 2 cups
a day is fine
7. Eat in small frequent portions at an interval of 2.5-3 hrs per meal, chew slowly with small bites
8. Drink a lot of water (3-3.5 ltr per day), stay well hydrated
9. Have your dinner at least 2.5-3 hr before your dinner time, take a brisk walk after dinner
10. While sleeping, try to keep your head end a bit higher than usual, using a bigger sized pillow will help..
Most importantly, don't take stress and avoid stressful situations(as much as possible)
Follow all these and Report back to me after a few weeks about how you stay..
With all these, you should be feeling better within a matter of time only, but you have to maintain them, that's extremely important..

My doctor saw those reports and initially he wrote
Razo -D before breakfast
Razo -20 before dinner
He told me to continue using these two for another two weeks
and gradually reduce dinner tablet and later first one also
even if there is discomfort after that he'll start some other **** therapy
The report shows something chronic , Is this treatment enough? are these tablets powerful enough?
I'm scared after you told all these dots are ulcers!
i still don't understand why a guy like me who doesn't even drink got all those?
It is not even caused by bacteria as mentioned in biopsy!
Can gluten(wheat) allergy cause this?
According to my situation and availability I can have breakfast cereal(Muesli)
with milk and some fruits Is this a good breakfast for my condition?
Kindly help me with these questions

As I've already mentioned in details , you don't need to worry, so just relax..
Yes you can continue Razo as already advised by your doctor, it's a good medicine..
Report of Endoscopic biopsy of stomach showed - "diffuse chronic inflammation"- which is diffuse- involving the entire stomach region , chronic - persisting for long duration , inflammation - painful swelling along with redness and heat, I've explained this one also in my previous answer..
It's not really that 'scary' as these are all merely superficial ulcers, which, if you maintain and follow your Doctor's and / my advises along with the medications, will get healed within a matter of time only..
Drinking is not the only cause for these, there's a whole bucket list of causes for these, yes gluten sensitivity can trigger Acid-peptic disease, which, if left untreated, in long term can cause these..
If you can tolerate milk, then it's fine, but i'd advise you to consume Double toned milk only..
However, if your gluten sensitive, then please get in touch with a Dietitian for a complete Diet plan (mentioning this gastric ulcers, your height,weight,daily activities,food preferences etc.), then it'll be more suitable for you in the long term..

I'm feeling nauseous sometimes after a meal with bloating
I read somewhere gastric cancer manifests with similar symptoms
What does the biopsy tell regarding this? what is your opinion?

Feeling nauseous with bloating after meal is very much understood in your condition, so just go back to the very first answer of mine, and follow all those measures explained, you'll feel better..also try to implement the diet habits and lifestyle modifications which i've already mentioned, the recovery will be faster..
To tell you frankly XXXX, Gastric cancer manifests with every single symptom of yours, and a whole list of several other non-specific symptoms which can be manifested in another several diseases..
That's why we, the doctors, take history; evaluate the patients; run investigations and manage accordingly and also prognosticate for future well being too, it's a complete process..
Your totally clean, in this matter, so no worry.. just one thing, tell me if there's any history of gastric cancer in the family or not ? if yes, then i'll tell you few precautionary methods for future, if no then sit back and relax..
Once again i'm repeating the biopsy report for you -
"No definite H pylori, intestinal metaplasia, glandular atrophy or dysplasia or
malignancy noted in the section studied." - means there is no offensive bacteria, no doubtful changes in the intestine, no tissue shrinkage, NO Cancer noted in your biopsy sample..
Next follow-up will be 2 weeks after starting the medications along with complete dietary and lifestyle modifications as already explained (you won't get complete result out of it if you don't follow this whole process, it's extremely important).. if you feel anything too serious, you can follow-up even before then..
