What Does This Blood Report Indicate?

Question: I feel good . I have no symtoms . No swelled glIbrutinb capsules when ands or enlarged Liver , or Spleen. MY last CBC read Platelets 118, HGB 13.9 ,WBC 27.4 I am current doing watch & wait
for the CLL. one of my Doctors said to concider Ibrutyinb capsules when WBC reaches 30,000 another says to waite on the Chemo. Since I have no systoms . Whats your Opinion ?

Generally in my practice I do not necessarily treat based on the white blood cell number alone. The decision to treat is most often based on associated symptoms and other findings on the blood count including hemoglobin and platelet count. If there is a significant decline in the hemoglobin or platelet count this might be a consideration to pursue treatment.

in your case her hemoglobin is adequate. Your platelet count is mildly decreased related to the ITP and should be watched closely over time. I do not necessarily think treatment is indicated again based purely on the white blood cell count. If your platelet count begins to decline further or if you develop anemia or other symptoms including swollen lymph nodes, fevers or uncomfortable night sweats then your doctor can also consider initiation of appropriate therapy.

Are you aware of any new treatments being developed to treat CLL . Do you feel Ibrutinb capsels is a good choice to treat CLL are their any harsh side affects?

Inrutinib is fairly well tolerated and may be a good choice for treatment if you fit the criteria for front line therapy with this drug.

Chemotherapy also is very effective and generally well tolerated. As always we recommend discussion of specific treatment decisions with your primary hematologist.
