Question : Hi Doctor,

Result of some lab work I had done yesterday is attached. Should be two pages. Yes I know cholesterol on first page needs to get better, but my question is on blood count on 2nd page. I am in Thailand now and they referred to to hematologist, which I am seeing tomorrow. Doctor said something is going on in my bone marrow and red blood cells. Do you have any idea what you think it might be based on this lab attached work? Or what questions to ask Doctor tomorrow? What other tests might you run? Should I be concerned? Also, lymphocytes are low on this attached report. And my Lymphocytes seem to be trending downward as lymphocytes were 1447 (reference range at US lab 850-3900) back in November 2015. Not sure why Thailand has lymphocyte reference range of 1500-4000. Anyway, is my lymphocyte drop of any clinical significance as it is low on attached Thailand lab reference range? And no I don't have HIV. I just tested negative yesterday. Could the low lymphocyte thing be connected to bone marrow thing? One other thing to tell you, I have a swollen lymph node under my jaw. Been swollen for four months now. At least thats when I first noticed it. Doctor at Thailand hospital confirmed it was swollen and wants hematologist to look at it. Also, I had a big jump in neutrophil count from November 2015 to lab work attached here. November 2015 neutrophil count was 3553 (reference range 1500-7800) and attached here neutrophil count is 6045. Is that of any clinical significance? Are all these things connected? Please advise or let me know if you have any other questions. Over all health in general is good, no fevers, no fatigue, no diarrhea, no coughing, no bizarre weight loss, no other lumps/bumps on me other then lymph node under jaw, but I would say last 6 months I have had very odd skin issues to include folliculitis and odd rashes in armpits from time to time. Keep in mind prior to that I have had pretty much perfect skin at my life. Anyway, thoughts? Any other questions for me?

I have gone through your question. I can understand your concern.

Your hemoglobin is slightly high with high normal RBC count. It can be due to polycythemia vera. (chronic myeloproliferative disorder with high RBC production in bone marrow). Or some other cause like dehydration.

In my opinion you should repeat your complete blood count after two weeks. If you have persistent high hemoglobin then only you need further testing for polycythemia vera.

Your neutrophil is high and lymphocytes are low but it is not significant much. It can be due to normal variation or some sub clinical infection. So don't worry about that.

You also have enlarged lymphnode. For that you should take a course of antibiotics. If it remains persistent after treatment then biopsy diagnosis is advisable.
