Question: Hi, I am a 69-year-old female I've been feeling quite ill for a while and my doctor decided to check my thyroid. My TSH has been 0.01 for the last two months. He did some other thyroid bloodwork such as T3 and T4 which were normal, however my thyroid antibodies were high. Thyroid peroxide about is 1285.0 and Thyroglobulin antibodies are 21.8. What is wrong with me. I also was dx'd 2 yrs with an ANA of +1:5012.

Brief Answer:
I would recommend some more exams.

Detailed Answer:
Hello,
Thank you for asking on HCM!
I read carefully your concern.

From your hormone results it seems to be sub-clinical hyperthyroidism or a slight over active thyroid.

The high titre of the antibodies is suggestive for an autoimmune disease called Hashimoto's thyroiditis that causes an under active thyroid but in rare cases in it's early stages it is manifested with an overproduction of the thyroid hormones that does not last too long and is than substituted with a permanent under production.

But, I would suggest you to check Anti TSH receptor antibodies that cause an over active thyroid disease. If they result to be high than treatment is suggested.

I would also recommend to do an ultrasound of the thyroid gland to see if there is a nodule within the gland that over produces thyroid hormones.

So you need to rule out 2 other diagnosis, which needs treatment if confirmed, before you decide for a transitory phase of Hashimoto's thyroiditis (Tachi-hashimoto) that does not need to be treated.
Hope I have been clear and helpful.

Best wishes,
Dr.Mirjeta
