What Does This Blood Report Indicate?

Question: I can only walk about 10 steps and I'm very short of breath. Chest hurts as I often struggle to get a breath. My left lower leg is painful to the touch. Right leg also sensitive, but not as much. This has been going on about 10 - 14 days. I had blood work on the 9th of Dec. transferrin is 377. iron is 21, iron sat is 4, ferritin is 12.26, vit D 11.8. What is going on and what should be done?

I had a pulmonary embolism in March of 2015. I am not on any blood thinners anymore.
