Question: Hello Dr.
I am 25 years old woman, got married 3 years before,I used to take a contraceptive Dronis 30 but i stopped it 6 months before to get pregnant. I am taking folic acid123,gluconorm sr 1gm regularly.according to dr advice. 1 month before I took fertomid100 from day 2 to day 5 of my period. Follicular study has also been done with good result. I took fertigyn as well at the day 15. I am yet to get pregnant. Dr advised me to walk again through this cycle. Can you please provide me a second opinion on this?
Whether this is the correct path?
How long I have to go through this cycle?
What is the next step if I don't get pregnant for a long time?
Is there any thing need to be done from my husband side as well?
Is it ok to do some beautification treatment like hair straightening at this time?

For your information (shortly) my last follicular study report ..Day of cycle17 --endomet. thickness-9.5, right ovary-14.3*11.6, left ovary-22.3*16.9, (date 30.8.2014)

All queries have been address ma'am

Detailed Answer:
Hello ma'am and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. You have most probably been diagnosed with polycystic ovarian syndrome (PCOS). This condition normally causes a few abnormalities and affects the fertility of a woman.

Although very common today, and very commonly treated by doctors; some patients can take more time to respond to the prescribed treatment regimen. Let me described the treatment regimen prescribed to a woman who has been diagnosed with PCOS, for your better understanding.

-The first line of treatment is for women who have been diagnosed with the condition, whether or not they are trying to get pregnant. This is to prescribe medications to help regularize the menstrual cycles (which in almost all cases, is irregular). I am guessing your menstrual cycles also have been irregular, right?
+Although there are many different medications available in the market for the treatment of PCOS, the most popularly used one for the treatment of PCOS today is metformin. The reason why this medication is of growing importance and use is because it not only addresses the irregularity of the menstrual cycle and helps regularizing it, but it also promotes weight loss (which is quite important in most women diagnosed with PCOS, as most of them are trying to lose weight but are not succeeding). Your doctor has prescribed gluconorm for you, which is metformin

-The treatment using metformin is continued until a woman can have regular menstrual cycles without the help of metformin. The next change in the treatment regimen is:
+When a woman wants to get pregnant. If a woman with PCOS wants to get pregnant, then the first aim is to make sure her menstrual cycles are regular. Until they are regular, she is not allowed to continue with the treatment. Once they are regular, we prescribe a medication called clomiphene citrate (which is fertimid). Your doctor has given you this, but I home he/she gave you this only after your menstrual cycle was regularized. If not, then this is the reason why you are not able to conceive.

Usually after treatment with fertomid (to be tried for at least 3-6 months), most women conceive. But for those who do not, the following is done:
-hCG injections are added to the treatment regimen
-The male partners semen analysis is done to check if he is fertile
-If all is done and yet conception is not achieved, other procedures like in-vitro fertilization (IVF), gamete transfer (GF), intra-uterine insemination, etc; are advised.

Now that I have provided you with this information, in response to all your queries:
-Yes ma'am, this is indeed the correct path
-Usually for 3-6 months. Success is achieved within this duration of treatment
-As mentioned in the above explanation:
+hCG injections are added to the treatment regimen
+The male partners semen analysis is done to check if he is fertile
+If all is done and yet conception is not achieved, other procedures like in-vitro fertilization (IVF), gamete transfer (GF), intra-uterine insemination, etc; are advised
-Your husband should have a semen analysis conducted. This should be done in all couples, whether or not they are struggling to conceive
-Yes, it should be fine to go ahead with the beautification

I hope you find my response both helpful and informative ma'am. Please feel free to write back to me for any further clarifications, I would be more than happy to help you.

Best wishes.
