What Does This Following Blood Report Indicate?

Question: Hello, I have a 2.5 year old with history of lymphocytosis and monocytosis, now both are normal. He has also had high RBC, RDW, alternating normal or high hgb and hct, and alternating barely normal to low MCV and MCHC. Is this a type of anemia that needs to be treated?

His most recent RDW was 18.8 and RBC was 5.53; no smear was completed.

I can understand your concern.

His hemoglobin is low normal for his age. It is not a cause of worry. Slight low hemoglobin is common in pediatric age.

But he has slight low iron with hypochromic RBC in smear. So mild iron deficiency is there which can be treated with dietary supplements.

lymphocytes and monocytes are normally high in pediatric patients. So it is normal and no need to worry about that.

Don't worry about RDW and MCV and other indices. It is not significantly abnormal.

So just give him iron rich food. Otherwise his hemoglobin is quite good. Just be relaxed.

So we have been adding a variety of beans and red meat to his diet every meal for the last four months and he is still very fatigued. He eats spinach and vegetables very well and his twin sister and older brother (1 year older) do not have the systems he has, but they eat the same diet....hence a comparison and reason to be concerned. I know every kid is different, but he has even stayed asleep during an allergy skin test (that he reacted to). What else could be causing his fatigue if it isn't anemia?

Most of the time anemia is not the cause of persistent fatigue.

I can understand your concern.

Mild anemia never cause persistent fatigue. Severe anemia can leads to fatigue.

In your case fatigue must be due to some underlying other cause. Viral infection, tuberculosis, diabetes, inflammatory bowel diseases, chronic kidney disease, sleep disorder, allergy or asthma can leads to persistent fatigue.

In my opinion he should be examined by pediatrician thoroughly then investigates accordingly. Find out the cause and plan accordingly. No comparison with siblings or others because each child has different health issues.

But don't worry about anemia. It is not the cause of his fatigue.
