What Does This Histopathology Report Indicate?

Question: Hi doctor,
I was pregnant for almost 8 weeks. As doctors did not get baby's heart beat (Yolk sac and Fetal pole as found from TVS USG), They decided to go for D&C and examine the sample for biopsy test. Kindly let me know what was the exact abnormalities with my pregnancy as doctor referred as missed abortion.
From the Hystopathology repot of Microscopic Examination shows the following results :

1. Sections shows decidual tissue and blood clot,
2. However, no chorionic villi is present,
3. No molar tissue was seen,
4. No miligancy is present

Greetings
I am sorry to know about your missed abortion. But not to worry.
It confirmed that you can conceive again and there was some abnormalities with the fetus hence got aborted.
Now coming to the biopsy report. It is showing only the products of conception. Another possibility is to rule out molar pregnancy,which is rarely associated with conception. Since you don't have it , nothing to worry.There is no abnormalities with your biopsy .It was from the specimen of missed abortion.
You can try for next pregnancy after 2 normal cycles.

Thanks for your nice feedback doctor. Can you please tell me that not having Chorionic villi in the biopsy report means some abnormality? What does it mean if Chorionic villi was present?

What kind of abnormalities does the fetus have? What kind of precuation I have to take for next pregnancy? I did not miscarriage naturally. Was d&c the right decision? Was it possible to get the heart beat after few weeks as the biopsy report seems ok?

Also please be informed that my few USG report results varies from one another and some error from those reports were found.

Hi
Chorionic villi will be seen after few days or weeks of pregnancy.Once the embryo grows ,it takes its nutrition via Chorionic villi from mother.I have simplified for you to understand.Since there was no cardiac activity doctors are justified in doing d&c as continuing with out heartbeat and doubling hcg is not advisable.
Scan measurements can vary ,but fetal cardiac activity is very evident.
Coming to precautions.Nothing much.same as how you were now.
It is not because of lack of precautions. Usually chromosomal abnormalities are seen in such abortions.
Good nutrition , proper sleep ,yoga and meditation Avoiding junk foods,carbonated drinks etc will support.
Folic acid tablets has to be taken.

Can you please tell from when I can start taking folic acid?

I had d and c on 28 July 2016. Can I take heat pad or caster oil pack to relief from the pain of Endometriosis?

From when I can start my job and normal life after d and c?

I used to consume flex seen soaked water in the morning wirh empty stomach before my pregnancy, shall I start it again?

Does fish oil (omega 3 fatty acid) capsules help me in this issue? Please tell me about its' doases

Hi

You can take folic acid from now onwards, it will prevent nueral tube defects for your next baby. Ideally you can take from 3 months before planning.
You can resume your work after 24 to 48 hrs of D&C.
Also can use heated pads for pain.
Regarding flax seeds and fish oil, no scientific evidences as far as I know. Fish oil can have unwanted side effects. Please go through some scientific references regarding items mentioned.

Subject: Need suggestion on medicines

Dear doctor,

Thanks a lot for your nice suggestion, I am convinced. Kindly find the enclosed doctor's prescription and have a look at the following medications I have been taking:

Phase 1. From 14 XXXXXXX to 16 July 2016:
1. Proluton Depot Injection (one time)
2. Tablet Duphaston 10 Mg (1+1+1),
3. Omidon (1+1+1),
3. Pantobex (1+0+1) before meal,
4. Aristovit (1+0+1),
5. Folgyn (0+0+1)

Phase 2 Medicines: I have consulted with another doctor and taken these medicines from 17 July to 28 July'16
1. Microgest (100 mg) for 1 + 0 + 1 . two times daily
2. Aristovit for 1 + 0 + 1 . two times daily
3. Folic acid. once daily (at night)
4. Pantobex (20 mg) for 1 + 0 + 1 . two times daily before meal for acidity

Phase 3 Medicines after D&C- Surgery was done on 28 July and I taking these medications (no. 1 and 4) till now:
1. Antibiotics neofloxin 500 mg (1+0+1) for 21 days,
2. Filmet 400mg (1+1+1) for five days,
3. Tab Lexotanil (0+0+1) for 7 days and
4. Vitamin Pregnova as vitamin and mineral supplements (0+0+1)

Phase 4: Medicine will be taken at the first day of next period
1. Tab Normon (5mg) 0+0+1;
2. Tab Letrul (2.5 mg) 1+0+1;
3. Tab Calcin D (500 mg) 0+0+1

Kindly let me know is my current medication and the medicines I will take after having my period back is okay? As I am having problems with Endometriosis, do I need to continue Duphaston or Microgest? As doctors did not suggest folic acid, I am bit confused. It will be great if you share an updated list of medicines that I need to take at present and at the time of my period.

Still I am having pain and bleeding at the lower abdomen if I carry heavy staff, also having pain at the area of lower abdomen where surgery took place and I am having period (slight) till now.

Looking forward to getting your valuable advice.

I was going through your list of medicines.some of them I couldn't find out.
Now for your endometriosis,duphastone has to be taken ,however if planning for pregnancy you should not be taking it
Folic acid can be continued even now.
Your future fertility management depends on so many factors.
conditions of ovary.AMH level ?
conditions of uterus and patency of tube ? Patency will be normal as you conceived now.
We cannot make a protocol with out knowing all details.Only your treating infertility specialist will know all details.
Hence I suggest you to consult a infertility specialist and get yourself evaluated and pregnancy may be planned after one or 2 cycles.In endometriosis it is better to plan pregnancy at the earliest.

Dear doctor,

Many thanks for your kind suggestion, these seem really helpful for me.

Can you please tell me how many times I have to take "Duphaston" tablet each day, what will be the dose?

Also, I used to take folic acid (5 mg) once daily. Right now I am taking multivitamin and mineral tablet named "Pregnova", where folic acid level is 400 mcg. Can I take additional folic acid and please tell me about it's dose?

Can you please tell me how can I know about the condition of overy AMH level? Is there any test? Kindly suggest me some medical test to know about my fertility condition (hormone test could be).

Kindly suggest me about the following medicines that doctor wants me to start when I got my period back again after D and C:
Medicine will be taken at the first day of next period
1. Tab Normens (Norethesteron) (5mg) 0+0+1;
2. Tab Letrol (Letrozole) (2.5 mg) 1+0+1;
3. Tab Calcin D (500 mg) 0+0+1

As my husband is living abroad, it will be difficult to plan for pregnancy for the next few months. Thanks anyway for your kind feedback.

Greetings. sorry for the delay.
Blood levels of AMH can be tested in endocrinology lab along with day two ultrasound scan to know your antral follicular count.
Regarding duphastone and the drugs mentioned are to be taken according to the severity of your endometriosis. I won't be justified in prescribing hormones for you without knowing full details , as it varies from person to person.
Duphastone can be taken from day 5 to 25.But you have been asked to take letrozole now.I think you should be discussing with your specialist regarding your husband's absence now.
That is why I suggested to consult an infertility specialist.
