What Does This Lab Test Report Indicate?

Question: My mother reports are showing eGFR= 21 mL-min-1.73 and UREA NITROGEN=24.8 mg-dL and CREATININE= 2.23 mg-dL. She is hypertensive, had stroke, feet are extremely swelled. Is she in dangerous range? What treatment is required? What medicines needed to be taken?

Thanks for the query

From the reports you have attached, it looks like she has renal impairment, probably secondary to hypertension. This is not in the dangerous stage of end stage renal disease as of now. Here is how you should go about it

1. Make sure to keep blood pressure under good control of less than 140/90 mmHg
2. I recommend that she is on ACE/ARB group of drugs like Enalapril, this increases the creatinine by 10-20% but provides long-term benefits,
3. She must avoid fruit juices, tender coconut water and fruits rich in potassium like Banana
4. Makes sure she restricts salt. Avoid food rich in salt like pickles, chips, etc
5. Her fluid intake should be less than a litre per day

It is correct that she is in Stage 4 Kidney disease. She can continue to take the medication she is on. I recommend Tab Lobun for her along with Cap Becosules as supplement. Lobun is a probiotic which is known to reduce creatinine and Becosule is a multi vitamin tablet.
I strongly recommend that she is on ACE/ARB group of drugs, it has been proven time and again that ACE/ARB group of drugs retards the progression of kidney disease. Please speak to your doctor regarding this

Thanks for getting back. I am sorry for the short delay in my reply I somehow missed the follow-up alert.
The waiting list for the transplant is not regularised in XXXXXXX Each state has its own set of rules, for example in Karanataka one can go on the list only if he/she has been initiated on dialysis. Pre emptive transplant cannot be deceased donor transplant.

However its too early to think about transplant in your mother's case. The aim should be to prolong the need as much as possible by the suggestions made by me in the earlier post
