What Does This Protein Electrophoresis Report Indicate?

Question: If I came back ANA by Hep2 Cells Quant. positive with a nucleolar pattern but negative on "ANA comprehensive Plus Profile (Labcorp)" which is the more accurate measurement? I am 31 with Stage 1 arthritis in all major joints. I tested positive for HLA DQ 2.5. I have been diagnosed with three different Anemias- Iron deficiency anemia, Pernicious Anemia, "other Folate Anemias". Will be doing a Iron intravenous infusion this week and B-12 shots for life most likely, and taking Folate to raise it from 3. Not Vegan or following any strict fad diets. I was told Pernicious Anemia is rare at my age, as is Folate Deficiency. Any of these connective?

I have gone through your history and the protein electrophoresis report....Just to summarize your symptoms:

1. Arthritis
2. Anaemias( PErnicious, Folate, Iron deficiency
3. ANA positive
4. HLA DQ2 positive

Well, Firstly all these conditions are autoimmune, that is your body is attacking your own body...And ANA is a marker for autoimmunity...SO it can be positive even in Celiac disease and Pernicious anaemia. As for Gluten sensitivity, it is difficult to say only on HLA basis unless you have the symptoms.

I have like to know a few more symptoms before commenting further:

1. DO you have loose stools?

2. Is there any swelling in the joints?

3. ANy history of oral ulcers/Hair fall/photosenstivity?

4. Any history of weight loss/appetite loss?

5. Any history of rashes over the face??

6. Do you have symptoms of cough/shortness of breath?

DO let me know these details and I would guide you further from here..

I do have a history of loose stools, though it's become constipation more often than not. No swelling as far as I know, just a lot of stiffness. I do have oligiodontia so I have a mouth full of dental implants, only 8 teeth are my own. I do have photosensitivity, I have lost 17% of my body weight since January, haven't been trying just can't seem to eat more than a few bites without feeling extremely full.. yes to shortness of breath worsening when laying down and always "clearing my throat" cough. Is the protein electrophoresis normal?

Should also add platelets, WBC, RBC, RDW all mildly elevated. Reticulocyte count at 18,000. Complement 3 and Complement 4a very elevated. Haptoglobin, Homocystine, CRP, High sensity CRP, SED rate all very elevated.

Well, your symptoms do point towards an autoimmune disease going on...Something like an Overlap of a couple of diseases. Considering your joint pains, loss of teeth( which I think is due to dryness of the mouth), cough--> It does point towards Sjogrens Syndrome. You can have a normal ENA which can be a possibility.

If you could attach the blood reports, it would be helpful..I want to view how high are they and derive the significance from it.

Pernicious anaemia, celiac disease are other forms of autoimmune disease which can accompany Sjogrens Syndrome.

Your protein electrophoresis is mostly ok. There is increased alpha 1 fraction, which can be non senstive as it can be high in any inflammation.

I think we need to schedule an appointment with a rheumatologist and an endocrinologist to sort out the issue. MEanwhile if you could attach the reports, will be beneficial

I should have clarified that I was born without the permanent teeth, I didn't loose them, never had them to begin with! I'm going to attach these labs to see what you think of them.

I should also add, I know that Anemia is typically associated with low RBCs mine are high (not very but still always out of range). How would anemia be causing this increased in RBCs? Is any of it responsible for the enlarged liver?

Sorry for the delayed reply.. had A marriage function in the house and was occupied there..

I have gone through your reports...Some of them are do significant, but overall they are good...

I will just summarize:

1. Vitamin D levels--> Quite low--> It has to be above 40
2. Choletrol levels are all deranged
3. high C4
4. high Reticulocyte count
5. high Levels of haptoglobin
6. HLA DQ2 positive...
7. ANA positive ( low titre and negative in other)
8. Lot of IgG positive in the infections--> Which is a good sign...
9. Low Vitamin b 12 and folate
10. high CRP, high Homocysteine

1. If given a weightage, High C4. high Haptoglobin are nothing to be worried about. Low levels do call for a concern but the higher ones are ok to go with

2. I suppose you are on vitamin D supplements, which should recover with time
But yes the deficiency of Vitamin D can cause joint pains and stiffness, which gets better once the levels reach around 40 and remain sustained..

3. As such your haemoglobin is in normal range, so I wont call it anaemia. All though other cell fractions are low which is common with Low iron, folate and b12 levels. So I think it is more to do with malabsorption..Which brings us to celiac disease. Your HLA DQ2 is positive, which can go in favor that nutrients are not being absorbed properly.

4, My only concern is high reticulocyte count.. It normally indicates either loss of blood or due to vitamin Defeciency. Since your Haemoglobin is stable, you are not losing blood. But we should recheck it soon after

5. As for your ANa, it is a very weak titre and may not hold too much significance in your case.

So to conclude, I feel it is more of a celiac disease with malabsorption( Iron, B12 and Folate, Vitamin D). As for ANa positive, it can be positive in this disease as well..
