Question : hi, I am 31 years old unmarried, I had torsion of right testis in 2005. my local doctor did not perform surgery but the testicle has been reduced to size of peanut.recently got tested and found that having grade 1-2 variocoecele in left testis and semen analysis says count of 3million/ml, rapid progressive motility 2%no moility88%, normal forms 20%abnormal 80%,general surgeon advised me not to go for surgery but an urologist has prescribed maxoza-L two times a day for a month and followed by a surgery. I have to get married please advise what actions to be taken????

Thanks for your query ,based on the facts that you have posted it appears that you have your your Rt testis to be atrophied due to Torsion in 2005 and detected to have grade 2 Varicocele on left side .

Your semen analysis has revealed you to have Oligoasthaenospermia suggesting your sperm count and active motility of the sperms to be very low compared to standard WHO range .

To be true there are no medications that can increase sperm count and sperm motility .

In a given situation you can continue taking Maxosa as prescribed by your Dr and repeat semen analysis after three months .Surgery for Varicocele is successful in only 30% of cases to improve sperm count .

If there is no satisfactory improvement in sperm parameters you have to consult qualified Infertility specialist for further treatment .

You have to accept the reality and explain these facts to your parents before taking decision for getting married .

Dear sir,

Thanks a lot. sir I would like to know whether I should go for a surgery or not as per your experience. If I get married will it be liking betraying a girl. I have no problems with erection , I get slight pain during erections.somebody suggested for acupuncture treatment will that help?????? Please advice. what is the grading of variocoecle with respect to my condition

Thanks for follow up.

You have Varicocele of grade 1-2 .There is no 100% guarantee that your sperm count and motility will increase after surgery .

I have mentioned in my reply earlier that there are only 25-30 % chances of increasing these parameters after surgery .

You make these facts clear to your parents and the girl concerned before taking decision and take a informed consent before going for marriage .

However note that the science and technology in reproductive techniques and infertility are advanced so much that there are chances of conception even when the patent has azoospermia .
