Question: Hello.
Wondering what is up with my Thyroid. Recent blood work
TSH .02
Free T3 3.3
Free T4 1.1
The doctor told me possible Hashimoto, but isn't a TSH .02 Hyperthyroid?

Hello Madam,

I understand your query and concern.

Your lab values are suggestive of depressed TSH with normal Free T3 and T4 levels.

This suggests a clinical stage of Sub clinical hyperthyroidism.

This can be an initial manifestation of early stage Hashimoto's disease.

But having said that the overall clinical feature of Hashimoto's disease is Hypothyroid clinical state reflected as high TSH and low T3 T4.

Please note that early stages of hashimoto's presents as hyperthyroid stage followed by hypothyroid state in later course of disease.

So to resolve this discrepancy you need to undergo screening for Anti TPO (thyroid peroxidase)antibodies which are largely specific for Hashimoto's auto immune thyroiditis.
