Question : my tsh count was 0.09
my free t4 was 20 on last review.
i take 200mcg of thyroxine.

Depending on the units that are used to measure these tests, the normal ranges for Free T4 are 0.8-2.8 ng/dL
TSH are 0.3 - 5.0 uIU

If your lab values were reported using these units, then I can compare.

Your TSH, if measured in uIU is low.
Your T4 is high.

This means the dose of thyroxine you are taking is probably too much for you. When an excess amount of thyroxine is taken, then T4 (which is a measure of thyroxine level) is high. And when the thyroxine level is too high, the pituitary decreases it's production of Thyroid Stimulating Hormone (TSH) because it thinks the thyroid gland is making enough or too much.

I recommend that your follow up with your doctor who will likely decrease the dose of thyroxine you are taking, and then recheck in 3 months. Unless there is a reason your doctor wants you to be in a hyperthyroid state for a particular reason and has discussed this with you, this would be the usual course of action.
