What Does This Transabdominal Ultrasound Scan Report Indicate?

Question: Health issue: bloating (my tummy is big as if I am 4-month pregnant)
Extract from report from transabdominal ultrasound:

The liver (7.6cm) shows normal size, smooth contour and normal echotexture. There is no increase in echogenicity to suggest fatty infiltration. No focal lesion is seen.
The intrahepatic ducts and common bile duct (0.43cm) are not dilated.
There are few tiny gallbladder polyps, measuring up to 0.2cm. The gallbladder contains no calculi.
There is no abnormal gallblader wall thickening. No pericholecycstic fluid is present.
Portal vein is patent with nornal flow direction.

The head and the body of the pancreas appear unremarkable. The ail is obscured by the overlying bowel gas. Spleen (7.3cm) is unremarkable.

Both kidneys show normal size (R=10.3cm, L=11.2cm). There is preserved cortico-medullary differentiation.
There is no hydronephrosis, mass lesion or obrious calculi.

The bladder also appears unremarkable with no obvious stone or mass lesion. No abnormal wall thickening is observed.



Please advise any problem with me and what tests are required for further checking?

*How long you have Stomach bloating?
Ans: More than a year

*Whether you have any ano-rectal or perianal piles/ fissure like problems?
Ans: No

*Do you have any accompanied IBS with Constipation?
Ans: Constipation, have a poo every 3-4 days with Type 1 or Type 2 (according to Bristol Stool Chart)

*What's your diet style vegan/ or non-veg, spicy, fried?
Ans: Only 3 meals each day. For breakfast, mainly coffee/tea with bread rolls from Pret A Manger. Eat rice during lunch and dinner, mostly Asian meals, mainly steam pork, chicken, sometimes fish. Can't eat spicy food. Sometimes eat slightly fried food.

What are other complaints with stomach bloating?
Ans: Easily feel full after meal, but actually start to feel a bit hungry few hours later; and before the time for the next meal.

*Update any other reports/test like stool/ova/cyst, blood tests, LFT, etc?
Ans: No other reports and test.

* Give more details as to what is your health problems, and brief in show how it developed, what is its current status, what are current complaints
Ans: Easily fart slightly daily; and feel dull pain or very slight pressure inside lower abnormen (sometimes at left, sometimes right or sometimes middle). Each time the dull pain lasts for around 1 or 2 seconds. If I don't pay attention to it, I may not even notice the dull pain/slight pressure. This occurs a few times each day. I'm 46 and BMI is within normal range. My weight increased from 52kg to 58 kg from year 2010 to XXXXXXX 2016 (ie. 5+ years ).

*Do you have any colicky pains lately
Ans: No.

Based on the query facts,upon reviewing the USG reports and updates,your stomach bloating is correlated with Irritable Bowel with Constipation(IBS-C)
The correlation of stomach bloating with Gall bladder polyps, needs to be investigated and kept in MIND,while treating your case.
In the given situation-only cause for your stomach bloating could be IBS with Constipation.
After the updates, gall bladder polyps seem to be causing any contribution to your Stomach bloating.

Do's to treat Stomach Bloating-
Change dietary style.
Be on bland / liquid/ soft diet with Yoghurt helpings x 2 times x 2 weeks
Be on low food loads and reduce dietary stress
Control accompanied food induced Bowel infections with needed antibiotics and anti protozoals,as needed.
Probiotic-Cap Sporalc x 2 times x 2 weeks
Don't add solids veg/ or non-veg to intakes.
Tab Multivitamin-Like-Zincovit 1 x 2 weeks.

Lab tests-
Get Complete Blood test,Stool-Ova/Cyst / Anal examination from Surgeon-to assess for anal piles,fissure and treat them under guidance of Surgeon accordingly.

Evacuants for Constipation-
Movicol Sachet-for bowel evacuation if need be.
Tab Dulcolax 2 at night with plenty of fluid intake would be sufficient to take care of your chronic unattended Constipation, which would be corrected with in 1-2 weeks.

Next appointment-
Update Me after 2 weeks for further assessment and planning of treatment in your case.
