What Does This Ultrasound Report During 15 Weeks Of Pregnancy Indicate?

Question: Hi Doctor,
My wife( 30 years) is almost 15.5 weeks pregnant. She had a scan at 13 weeks 6 days and the doctor told that NT is normal(1.3) and the free beta hcg and papp-a values also came as normal. But she said that the scan shows negative a wave ductus venosus. Please let me know the chances of complications with abnormal dv flow but other things pretty normal.

Hello sir, I have seen the reports.
They are normal and just incidental finding of diastolic notch which is common before 16 weeks of pregnancy because placenta is less developed.
Get this uterine artery Doppler repeated at 22 weeks and see for notch.
In case notch is still present then you will need to add certain simple drugs on daily basis and a regular ultrasound and clinical check up.
Also (if notch persists in 22 week Doppler) then there are higher chances of intrauterine growth restriction and hypertension in mother.

Thanks for assurance madam but my doctor told me that there can be chances of defects if it has negative a wave in ductus venosus before 14 weeks...so we are worried so much and planning for 16 weeks scan to check the status. My wife is using ecosprin and folvite and progesterone supplements as of now..pls advise

Hello sir, yes this notch is definitely abnormal but wait for your next scan and then see for the notch.
It is common before 16 weeks because of premature placenta so nothing to worry.
Take the supplements as advised.
Ecosprin is very important and keep taking it till 34 weeks irrespective of the 16 week scan.

Hello madam, we have done 16 weeks private scan. PFA reports for the same. Still the notch exists but the radialogist said that it is improved than earlier. Please advise..and also GA is not 16 weeks compared to earlier scan.is this any growth restriction or normal?

I have seen the attached reports.

Yes the notch has decreased as expected and will decrease further with advancing gestation.
This notch if at all persists is only predictive of possible fetal growth restriction and maternal hypertension and if proper care is taken in the form of regular supplements and antenatal visits to the gynecologist then the outcome is usually favorable.

Also take a note of the following points -

- ask her to have a good rest with no exertion at all
- avoid coffee and tea in large amounts
- take ample amount of water
- ask her to take good amount of milk and green leafy vegetables
- almonds and walnuts are very good and should be taken on daily basis
- avoid any kind of stress
- keep taking all the medicines as prescribed daily without a miss.

The growth is alright already and a difference of ten days between ultrasound and gestational age by LMP is very common so just don't get anxious about the baby growth and weight at this stage.
