What Does This Ultrasound Report During 33 Weeks Of Pregnancy Indicate?

Question: hello Dr,
I m 35 weeks n 5 days pregnant.we did our ultrasound during 33 weeks 6 days and the results are
Fetal wt:2040 gms , placenta is anterior with grade III maturity ,cephalic position, adequate AFI 9.2 amniotic fluid ,internal OS closed and result is Gravid uterus with single viable fetus about 32-33 wks in cephalic position.
Last two days I ve got severe sudden swelling in my feet and went for check up and the Dr took urine test and I had 100 mg/dl Protein in my urine and my BP is 160/90.Is these symptoms I need to worry about and also can you please suggest me should I go for any treatments.

you have developed pre-eclampsia.

Detailed Answer:
hello,
thank for contacting healthcaremagic.

Your ultrasound is perfectly normal and the fetal growth is adequate.

But your BP and urinary proteins indicate that you have developed preeclamsia.
this happens in pregnancy when the bp rises more than 140/90mm hg.

this has to be followed up closely and pedal edema is a classical sign. you have to follow the mentioned protocol...
1. daily twice bp recording.
2. urine protein twice weekly.
3. tab labetalol 100mg thrice a day to start with and adjustment according to response.
3. foetal growth parameters by ultrasound 2 weekly.
4. non stress test and AFI weekly from now onwards.
5. antenatal corticosteroids administration for fetal lung maturity at 34 weeks.
6 aim to control bp less than 140/90.

to look for signs like epigastric pain, blurring of vision, dizziness....which indicate impending eclampsia , and can cause seizures. this has to be prevented. keep in touch with your gynecologist and if the parameters derange , you would have to be admitted to the hospital soon for consideration of an early delivery. though aim now is 37 weeks, pregnancy should be terminated at 37 weeks as it is considered safe for both mother and child. beyond that chances of maternal and fetal morbidity are high.

bp shall come back to normal after delivery as delivery is the only treeatment . however with good monitoring and follow up pregnancy can be extended to 37 weeks. NICU care may be required by the baby after delivery so keep that in mind.
so you have to be vigilant .
